    The following acknowledgements are part of your onboarding process. It is important to complete this form so that we have on file a receipt that you were provided the materials needed for you to be successful in role here at TCA. These documents should take you approximately 10 minutes to complete.
  • Acknowledgement of Receipt of Volunteer Handbook

    On your first day of volunteering, you will be provided with a TCA email account. You will receive a .pdf copy of this Volunteer Handbook and be asked to acknowledge the receipt of the Volunteer Handbook. By replying affirmatively to that email, you acknowledge that you have received this publication. Should you misplace your digital copy, you can acquire a new one from the TCA Intranet. Volunteers will be required to sign and date below annually in acknowledgment that they have read and consent to the current Volunteer Handbook.

    This section reaffirms your preferences and interest to volunteer in specific areas of TCA. These questions are a duplicate of the interest form you may have filled out online prior to onboarding and orientation.
  • Volunteer Conflict of Interest Form

    The standard of behavior at the Taiko Community Alliance (TCA) is that all staff, volunteers, and board members scrupulously avoid conflicts of interest between the interests of TCA on one hand, and personal, professional, and business interests on the other.  This includes avoiding potential and actual conflicts of interest, as well as perceptions of conflict of interest. I understand that the purposes of this policy are to protect the integrity of TCA’s decision-making process, to enable our constituencies to have confidence in our integrity, and to protect the integrity and reputations of volunteers, staff and board members. Upon enrollment, I will make a full, written disclosure of interests, relationships, and holdings that could potentially result in a conflict of interest.  This written disclosure will be kept on file and I will update it as appropriate. In the course of meetings or activities, I will disclose any interests in a transaction or decision where I (including my business or other non-profit affiliations), my family, and/or my significant other, employer or close associates will receive a benefit or gain.  After disclosure, I understand that I will be asked to leave the room for the discussion and will not be permitted to vote on the question. I understand that this policy is meant to supplement good judgment, and I will respect its spirit as well as its wording.

  • Confidentiality Statement -

    1) It is the policy of TCA that board members, volunteers, and staff of TCA will not disclose Level 1 or Level 2 information as defined in the TCA Data Use Policy, belonging to, or obtained through their affiliation with TCA to any person, including their relatives, friends, and business and professional associates, unless TCA has authorized disclosure or use. This policy is not intended to prevent disclosure where disclosure is required by law.

    2) Board members, volunteers and employees are cautioned to demonstrate professionalism, good judgment, and care to avoid unauthorized or inadvertent disclosures of confidential information and should, for example, refrain from leaving confidential information contained in documents or on computer screens in plain view.

    3) Upon separation of employment, service, and at the end of a board member’s term, he or she shall return or destroy, all documents, papers, and other materials, that may contain confidential information. Failure to adhere to this policy will result in discipline, up to and including separation of employment or service with TCA.

    4) Violation of the state statutes regarding confidentiality of records is punishable upon conviction by fines or by imprisonment or by both.

  • Recording and Photo Release

    The volunteer hereby licenses any photographs/video created while performing volunteer work for TCA under the Creative Commons Attribution ­NonCommercial 4.0 International License (available at http://creativecommons.org/licenses/by­nc/4.0/​). In summary, this is perpetual, non revocable license that allows anyone to Share (copy and redistribute the material in any medium or format) and Adapt (remix, transform, and build upon the material) the TCA photographs/video as long as they give Attribution (in a reasonable manner, appropriate credit and a link to the license and indicate if changes were made), and use the TCA photographs/video in a NonCommercial manner (a manner which is not primarily intended for or directed toward commercial advantage or monetary compensation).The volunteer gives TCA permission to record this work for the purpose of future viewing of work, and shall hold as permissible indefinitely.  Volunteer hereby releases and consents to the recording, reproduction and use of his/her image, voice, artistic or dramatic performance, actual or fictitious name, and any quotes or testimonials given by him/her in connection with the volunteer work through photograph(s), video recording(s), and/or audio recording(s) (the "Recording") by TCA, or its agents and employees, including any third party designee approved by TCA to produce or use the Recording. This includes social media or social media galleries that allow viewing and downloading consistent with the Creative Commons Attribution NonCommercial 4.0 International License. Volunteers acknowledge that although TCA has legal releases from event attendees, these legal releases do not extend to volunteer's permission to use others' likeness, and the volunteer will need to separately obtained permission from subjects if necessary for personal use.

  • Indemnification and Hold Harmless

    To the fullest extent allowed by law, Lessor shall indemnify and hold TCA and their Board Members, staff, agents, attorneys, volunteers, and subcontractors harmless against all liability, loss, damage and expense (including reasonable attorneys’ fees) resulting from or arising out of or related to TCA’s use of the equipment.  The provisions of the California Tort Claims Act, Government Code §910 et. seq., including its defenses and immunities, will apply to allegations of negligence or wrongful acts or omissions by TCA. To the fullest extent allowed by law, TCA shall indemnify and hold Volunteer and their Board Members, staff, agents, attorneys, volunteers, and subcontractors harmless against all liability, loss, damage and expense (including reasonable attorneys’ fees) resulting from or arising out of TCA’s own negligent acts, errors and/or omissions under this DRA or its performance.  The provisions of the California Tort Claims Act, Government Code §910 et. seq., including its defenses and immunities, will apply to allegations of negligence or wrongful acts or omissions by Lessor.
