THIS IS A ONE-TIME PERMIT. A ONE-TIME FEE OF $25 IS REQUIRED FOR REVIEW OF SITE/PERMIT Ordinance 1290.01

Site plan should include all property lines, structures, fences, and proposed area of enclosure, along with the dimensions and feet from property lines and structures, and shielding from street and neighbor properties.

BY SIGNING BELOW, PROPERTY OWNER AGREES TO:
Not create a nuisance; keep enclosure secure and clean; protect chickens, ducks and rabbits from threat; not sell eggs; comply with section 618 and section 1290.01 of the codified ordinances

AUTHORIZATION
Applicant certifies that all information provided is true and accurate and is submitted to include the issuance of a Certified of Zoning Compliance. Applicant agrees to be bound by all provisions of the Village of Minerva Park Ordinances, to complete work in accordance and comply with applicable Federal, State and Local laws and regulations, and to allow for the inspection of improvements outlined in the Certificate of Zoning Compliance application. Applicant hereby acknowledges that all work described herein must begin in six (6) months of the date of issuance and be substantially completed within on (1) year. Failure to do so shall void the certificate and no work may commence until a new Certificate of Zoning Compliance is issued.

AUTHORIZATION TO VISIT PROPERTY-Must be completed by property owner.

I, the owner or authorized owner's representative of the subject property listed on this application, hereby authorize Village representatives to visit, photograph and post notice (if applicable) on the property as described in this application.

AGREEMENT TO COMPLY AS APPROVED-Must be completed by property owner

The applicant of the subject property listed on this application, hereby agrees that the project will be completed as approved and any proposed changes to the approved plan shall be submitted for the review and approval to the Planning and Zoning Division staff.
