Welcome and thank you for your interest in Power of Praise Dance Academy where we go "From Passion to Purpose 8 Counts at a Time!" I am glad you are interested as you are taking the 1st step to understanding the true purpose of dance! We are a Christian Dance Company, so our focus is to worship and to show forth the glory of God through the movement arts! We will accomplish this by teaching you various dance technique including basic ballet & jazz technque! You or your student will learn various worship styles and how to incorporate various instruments like flags, billows, and glory hoops; just to name a few! Not only will you or your student learn the techical, hands-on side of movement but here at Power of Praise Dance Academy, you and/or your student will learn the biblical foundation side too! We did into where it started, the meaning behind various movement, colors, why we use various instruments, what it means to worship, and the list goes on...! My duty is not just to teach you but to also give you an understanding of "why?" I mentor in in a fun way! Whether you are male or female, young or old, even a beginner or you have been dancing most of your life, Power of Praise Dance Academy will give you add purpose to your dance because we don't dance to be seen, but that others will see Jesus!

I accept full financial responsibility for all related costs during the 2024-25 Power of Praise Dance Academy.

• These costs include registration, monthly tuition fees, garment fees, production fees etc..., and late fees (if applicable).
• I understand my tuition payment is for the entire month regardless of the number of days the student attends.
• If a student misses a day for any reason, no refunds will be given.
• I understand that payments are due by the 10th day of each month and a $10 late fee will be added to my account for any late payments.
• I understand that if my tuition is not paid by the 15th of each month, there will be another $10 added to the tuition. 
• Please be sure to communicate it to the Director if you think that your tuition will be late each month.
• Any student who wishes to withdraw from classes, MUST make it known to the Director, in writing 30 days in advance!

Always remember clear communication can solve a lot issues and assumptions, so be sure to communicate any problems that may arise as we know that unexpected things can happen. Thank you kindly!

During the course of the dance year, LEADance Alliance Ltd. Co., Power of Praise Dance Academy may wish to use photographs of Power of Praise Academy Students. The images will be used on the LEADance Alliance Ltd. Co Website, Facebook® page, and general media publications on a controlled basis and would highlight the student(s) demonstrating either learning techniques or participating in approved school activities. LEADance Alliance Ltd. Co., Power of Praise Dance Academy will not release individual names or "tag" with any photographs released.

I hereby give LEADance Alliance Ltd. Co - Power of Praise Dance Academy the absolute right and permission to use my and/or my minor child’s name, image, interview, or other auditory or visual image as a Power of Praise Dance Academy participant and copyright and/or publish, or use pictures, or videotapes, of me and/or my minor child of which the inclusion is in whole or in part, made through any media or Internet website at its studios or elsewhere, for research, education, advertising, trade or any other lawful purpose whatsoever whether taken in the classroom, prior to, or after class, at a Studio event, or a ministry event, dress rehearsal, or dance recital and whether these events are within, pre, or post the enrolled semester. I hereby waive any right that I may have to inspect and/or approve the finished product that may be used in connection therewith, or the use to which it may be applied.

Be aware that in registering yourself or your minor child in any dance or fitness class at LEADance Alliance Ltd. Co - Power of Praise Dance Academy (hereafter referred to as LDA - Power of Praise Dance Academy), you will be waiving and releasing all claims for injuries you or your minor child might sustain arising from participating in such activities.

I recognize and acknowledge that there are certain risks of physical injury to participants in classes at LDA - Power of Praise Dance Academy, and I agree to assume full risk of injuries, damages, or loss regardless of severity which I or my minor child/ward may sustain as a result of participating in any and all activities at LDA - Power of Praise Dance Academy. Understand that the staff exercises the utmost care in working with the students. Our goal is to provide a family oriented, safe and accident free environment; however, in the event of any injury and certify that I and/or my minor child are physically able to participate in classes and do hereby agree that this business, owners, employers, leadership, assistants or agents are not responsible or liable to me for any injury, accident, or loss of personal property. I, for myself and my minor child, do hereby release this business and its employees, leadership, assistants, owners and agent.s from any claim or cause of action which may have occurred as a result of participation in classes or as a result of any medical problem known or unknown of which I have knowledge presently or in the future. I agree to waive and relinquish all claims that my minor child or I may have against Power of Praise Dance Academy and its owners, agents, employees, ministry leaders, instructors, and assistants as a result of participating in classes at LEADance Alliance Ltd. Co - Power of Praise Dance Academy. I further agree to indemnify and hold harmless and defend LDA - Power of Praise Dance Academy, and its owners, agents, employees, leadership, instructors, and assistants from any and all claims resulting from inj uries, damages and losses sustained by me or my minor child arising out of, connected with, or in any wayassociated with classes at Power of Praise Dance Academy.

In the event of an emergency, I authorize Power of Praise Dance Academy or its agents to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for me or my minor child’s immediate care and agree that I will be responsible for payment of all hospital and medical charges
which are incurred. I recognize and acknowledge that Power of Praise Dance Academy or its agents will not be responsible for honoring specific hospital or healthcare provider preferences.

I have read and fully understand the above Waiver and Release of All Claims and Permission to
Secure Treatment.
