    Please fill the form below accurately to enable us serve you better. Submitting this form does not mean the confirmation or reservation.
  • Note: AMA needs two weeks to process and decide on this application. Kindly fill out this form beforehand to allow both parties sufficient time to process the request.

    No Last-minute requests will be entertained. 

    The person signing this form will be responsible for all liabilities arising from not being obliging to the property's terms and conditions or causing damage.

    No reservation will be confirmed until payment is made after the confirmation email.

    Following are the payments that need to be paid for confirmation:

    Rent is $150 per day (For Gym) (5 Hours)

    Separate rent will be charged  for hall (5 Hours) - No food is allowed in the hall else $200 will be deducted from the deposit incase this is violated.

    Gym needs to be booked for serving food($150 needs to be paid for this booking)

    Separate Deposit $500 for each hall and gym booking.(Any damage will be evaluated and will be deducted from this deposit)

    Cleaning Charges - In case food is served ($ 200) - This only includes mopping gym, cleaning washrooms, garbage disposal. 

    Cleaning Kitchen and returning gym to its original position is the responsibility of the person renting the facility, otherwise deposit $200 will be deducted from the deposit. 

    No cleaning charges for light refreshments (to go) are ok.

    Booking & Cancellation Policy for reservation:

    This policy aims to outline the standard processes, procedures and regulations for Al-Mustafa Academy rentals and bookings. The availability of the request will be reviewed, and the management will follow up with you in the next 2 to 3 days.

    - but it can take longer. Please keep 2 weeks' room while reserving the facility.

    Booking Deposit: A minimum of $50 non-refundable deposit is required on booking to confirm your reservation. The balance due will be shown on your confirmation & payment form, or you will be informed via direct message.

    We also need a refundable $500 damage deposit before confirming your booking

    Gym  $250 

    includes 10 round tables and 70 chairs

    Fully Equipped Kitchen

    A sound system is available

    Cleaning the gym, kitchen, and utensils used and putting them back is the responsibility of the person renting.

    Putting tables and chairs back in the storage is the responsibility of the person renting it.

    Partition curtain is available and is the responsibility of the person renting the facility

    Prayer Hall  $250 Mic System and chairs (for seniors) included

    Food is not allowed in the Prayer Hall - You need to rent a gym separately to serve food. 

    Takeaway food served outside the Prayer hall is ok - but it should not be eaten inside the hall
    Gym and Prayer Hall  $400

    All the above applies to the person renting the facility

    Additional Charges for:

    Need Chairs and tables: $100 (Someone will help you with bringing out the chairs & tables, setting up the chairs & tables, and putting the chairs & tables back in storage)

    In case food is served: $100 

    In case you are serving food and need tables and chairs as well: $200 (Someone will help you with bringing out the chairs & tables, setting up the chairs & tables, and putting the chairs & tables back in storage)

    If you are using the kitchen, bring your supplies.


    If we find too much of the facility is left messy, the green area is littered, or garbage is not disposed of in the garbage bin outside, $100 will be deducted from the deposit.

    Al-Mustafa Academy Guidelines

    • Check-in as mentioned on the form
      Check-out time 10:00 pm (max)
    • Quite time: 10:00 PM
    • No noise or gathering at parking lots.
    • Parking: Please Park appropriately to give access to the Firetruck and ambulance. (All fines will be incurred by the person reserving the facility)
    • Islamic dress is mandatory.


    Late Cancellation


    If you cancel less than 24 hours' notice, a $100 fee will be charged.

    DAMAGE and CLEANING POLICY: Al-Mustafa Academy is authorized to deduct from the deposit or charge the credit card if any damage is incurred to the facility or to the equipment during the event or if the facility is not returned clean and tidy.

  • This is an agreement between Al-Mustafa Academy Society (AMA) located at 5441 125 A Street, Surrey, BC V3X 1W4

    AND 

    1. Lessee(s) must be a minimum of 18 years of age and must be in attendance for the entire event.

    2. At the time of application, a $50 deposit for the center, or the total cost of the community or meeting room rental fee is required. The $50 is non-refundable, but may be transferred to another date.

    3. The account balance is due seven days prior to the event and shall be remitted to the AMA treasurer.

    4. The Lessee shall submit set-up diagrams to the facility and should contact the facility manager to make an appointment to review the set-up requirements if necessary.

    5. Rental items include set-up and take-down time. Should these times be overstayed, extra hourly charges will apply. The Lessor will have a representative on site that will check in periodically with the permit holder.

    6. The Lessee agrees to take all reasonable steps to ensure that the event is carried on safely and orderly and to ensure the safety of all people attending or working at the event. The Lessee shall ensure the entry doors to the facility are monitored by event staff or volunteers.

    7. For Al-Mustafa Academy Society facility rentals, Lessee is required to have general liability insurance with a minimum limit of $2,000,000. The policy must name "The Directors of Al-MustafaAcademySociety" as an additional insured and include a cross liability clause and tenant's legal liability insurance. You must provide this proof of insurance 10 business days before the first date of use.
    8. The Lessee agrees to place all garbage/recycling in the bags or containers provided and to clear tables, chairs counters and floors of debris at the completion of the rental.

    9. The lessee will be responsible for sweeping and mopping the floor, garbage removal and cleaning tables prior to and after the rental.

    10. AMA has no responsibility for personal injury or damage or for lost or stolen articles of the permit holder, or anyone attending the function.

    11. Failure to comply with the Facility Rental Agreement may result in the event being shut down and loss of future permitting.

    12. The Lessee shall ensure that smoking in the Facility is not permitted.

    13. The Lessee shall not suspend items from the ceiling. Only free-standing or tabletop decorations may be used.

    14. The Lessee shall not decorate the facility unless they have prior approval.

    15. The Lessee may not use tacks, screws and other tapes to affix objects to walls, windows, floors, doors, tables, chairs and ceilings and may not use tape on the floors. The Lessee must remove all decorations and adhesive materials at the end of the event.

    16. Candles are permitted if they are used on a birthday cake or wholly enclosed in a nonflammable container such as hurricane glass. The Lessee shall ensure that candles are not left unattended and are safely disposed of.

    17. The Lessee shall not play music, whether recorded or performed.

    18. The Lessee is responsible for paying any damages to property, buildings, furnishings, supplies or equipment.

    19. The Lessee shall comply with the facility’s Fire Safety Plan as follows;

      • Make note of all locations of Fire Pull stations and Fire Extinguishers.

      • Make note of all fire exits. DO NOT BLOCK EXITS. All exit points must be kept clear at all times.

      • Prior to the start of your event, inform occupants/guests that in the event of a fire, they must evacuate from the nearest exit to the designated safe area and remain on site until otherwise directed by the Fire Department.

    20. All deposits for centers are non-refundable ($150). Lessees renting the facility for a single event date will be permitted to terminate the rental agreement if notice is received 7 days prior to the rental date.

    21. Lessees renting for a single event will be permitted to cancel the permit for meeting rooms if notice is received 7 days in advance.

    22. The lessees agree that “illegal activities including assembling to conspire, propagate or incite hate and/or violence against other persons including ethnic or religious people are condoned and not permitted”.

    23. The Lessee agrees to observe, comply with, keep and enforce all applicable laws and regulations, all insurance and related documents, and all rules, regulations and by-laws of AMA, its Police and Fire Departments, and Provincial or Federal, but not limited to, matters affecting sanitation, health, fire prevention, safety, noise and crowd control.

     

    STATEMENT OF RESPONSIBILITY

    As sponsors of this activity, we agree to abide by the university policies and procedures that apply to use of this space. We assume the responsibility of clean up and guarantee payment for any damages. We understand that no food or beverages are allowed in classroom buildings.
