• Yappy Hour Form

  • *This form should only be completed if you have already been established as a client with shaffer animal hospital but this is a new pet to your household. If this is your first visit to shaffer animal hospital, please complete the new client form instead*

  • RULES

    1. Eligibility: All puppies under 5 months old are eligible for Yappy Hour. Puppies or dogs over that age will need to be evaluated by a trainer prior to attendance. It is strongly recommended that all puppies under 5 months old attend Yappy Hour consistently to benefit fully. All pets must be pre-registered in order to attend Yappy Hour. It is recommended to have your pet spayed or neutered (unless they are under 6 months of age). Intact dogs are still able to participate in the Yappy Hour sessions and are subject to behavioral assessment should the need arise.
    2. Payment: All payments must be made prior to attendance to allow you and your pet to take full advantage of the hour as soon as you arrive. We can safely keep your card on file as well if you choose. Payments will not be accepted the day of the event and is a cashless event.
    3. Vaccine requirements: All dogs must have up-to-date vaccinations. If you are a current client of Shaffer Animal Hospital and/or G.O.D.O.G.S. we will have your pet’s vaccine records on file. Otherwise, owners must submit vaccine verification from a Veterinarian showing current Rabies, DHPP, and Bordetella vaccinations as well as a negative fecal test. Puppies should receive a minimum of one set of vaccines and a first deworming at least 7 days prior to their first day at Yappy Hour session. If you are an established Shaffer Animal Hospital and/or G.O.D.O.G.S. client, you will receive reminders from us when vaccinations are due. It is your responsibility to provide on-going verification of current vaccinations.
    4. Health: All dogs must be in good health. Owners will need to certify that their dogs are in good health and have been free from any condition that could potentially jeopardize other canine guests or human staff. You are required to inform the staff if your pet has exhibited any of the following signs: coughing, sneezing, vomiting, diarrhea, lethargy, etc. Dogs that have been ill with a communicable condition in the last 30 days will require a veterinarian certification of health to be admitted or readmitted to Yappy Hour sessions. All dogs are expected to be free of fleas and ticks and we highly recommend a flea and parasite preventative, excluding flea/tick collars. If a pet is determined to be unfested with parasites, they may be asked to seek medical attention before return.
    5. Collars: All dogs must be wearing a quick release collar or a body harness. No choke collars.
    6. Check in & Yard Entry: Before entering the Yappy Hour yard, you must check in with a Doggie Daycamp Counselor. You will then be instructed at which entrance you and your dog should enter. So that no dog is overwhelmed by the volume of dogs upon entering the gated area, dogs must enter on leash, and remain on leash until the owner has walked them to the center of the yard. Once a trainer instructs you to let your dog off leash, you may do so. Additionally, when a dog and owner enters the gated area, you must leash up your dog until the owner and dog have entered the yard, approached the center of the play area, and the new dog is off leash.
    7. Potty Bags: Potty bags are not provided by our Counselors. Please bring your own, and clean up after your dog.
    8. Rainy Days: In the event of inclement weather, we may hold Yappy Hour inside. These sessions are restricted to neutral dogs and dogs under 40 pounds. Weather updates and closures will be made via email so please confirm we have an up to date email address on file.
    9. Appropriate Play: Some dogs play rougher than others. Please be mindful of how your dog is interacting with other dogs. We have Daycamp Counselors available to help you with your dog, but if your pup needs to be interrupted, redirected, or given a time out, it is the responsibility of the pet guardian to do that. If you require more assistance, we can schedule a one-on-one session with you and your dog. Please understand that if we recommend that you leave early on any given day, it is because we think it is best for the safety of all dogs. Please bring treats and be ready to work with your dog!
    10. NOTE: It is normal for some dogs as they mature to “grow out of” the Yappy Hour environment. If your dog has altercations that frighten or otherwise disturb the other attendees, he or she will be asked to leave. This does not necessarily mean that your dog should never play with other dogs. Many dogs who are not suitable for Yappy Hour or any dog park environment do just fine with one or two other dogs at a time that they know well. Any dog that instigates an altercation once and causes injury will be asked to leave, and will need to be evaluated as to whether or not it is safe for him to interact with other dogs at Yappy Hour in the future. There will be no refunds for dogs who are asked to leave.

    1. Owner agrees to release, indemnify, and hold harmless Shaffer Animal
      Hospital/G.O.D.O.G.S from any and all manner of damages, claims, loss,
      liabilities, cost of expenses, including reasonable attorney’s fees and related costs, arising out of or related to Shaffer Animal Hospital/G.O.D.O.G.S’ services, including, without limitation, (i) any inaccuracy in any statement made by yourself or information provided by you to Shaffer Animal Hospital/G.O.D.O.G.S’ (ii) your pet, including but not limited to destruction of property, dog bites, injuries, and transmission of disease, and (iii) any action by Owner which is in breach of the terms and conditions of this agreement. The Owner further agrees to be solely responsible for any and all acts or behavior of pets while it is in the care of the Shaffer Animal Hospital/G.O.D.O.G.S. Owner agrees to be financially responsible for Shaffer Animal Hospital/G.O.D.O.G.S’ property damaged by pet.
    2. Photographs, videotape or digital recordings are taken of Shaffer Animal Hospital and G.O.D.O.G.S. patients, clients, and staff on a regular basis for, among other things, use in advertising. I acknowledge that all such images (electronic, negatives and positives), together with prints and the copyrights therein, are the property of Shaffer Animal Hospital and G.O.D.O.G.S. I give Shaffer Animal Hospital and G.O.D.O.G.S. and persons authorized by Shaffer Animal Hospital and G.O.D.O.G.S. my consent, permission and authorization, without compensation to me, to use, reproduce and alter the images, in print or electronic format (including on the internet), either alone or in combination with other images, text and graphics. I waive my right to approve the finished photograph, advertising copy, print material or electronic files that may be used in conjunction with the images. I represent that I am at least 18 years of age and acknowledge that the rights granted in this paragraph are irrevocable, worldwide and perpetual, and are binding on my heirs, successors and assigns.
    3. I certify that my dog(s) are in good health and have been free from any condition that could potentially jeopardize other canine guests or human staff.
    4. To my knowledge, my dog has never guarded or become protective of food, treats, or toys around humans or other dogs.
    5. My dog has never harmed or shown any aggressive or threatening behavior towards any person or any other dog(s).
    6. This Agreement contains the entire agreement between the parties. This
      Agreement shall be governed by and constructed in accordance with the laws of the state in which Shaffer Animal Hospital/G.O.D.O.G.S is physically located and each party consents to the exclusive jurisdiction if the said state. Furthermore, the parties agree that any legal actions shall be subject to the venue of the court of competent jurisdiction in the county in which Shaffer Animal Hospital/G.O.D.O.G.S is physically located.
