• The 2023 TAP Health Summer Academy is a group of career exploration programs designed to build the next generation of healthcare professionals! These programs are TAP HC, TAP MD, and TAP RN. To learn more about each, visit taphealth.healthcollab.org.

    • This application allows you the opportunity to apply to one or more of these programs, using one form. If accepted, you will be in one program only. 
    • To apply, a student will need to be a current high school sophomore or junior in the Greater Cincinnati, Northern Kentucky, or Southeast Indiana area. 
    • Application in full is due on or by March 10, 2023.

    • Please Note: As TAP Health Summer Academy events take place in-person within health systems and organizations, ALL accepted students will have to provide documentation of COVID-19 vaccination to participate.     • The next 3 questions in this section relate to demographics. The information helps us collect data for our evaluation records, and will not affect your selection into the program. The questions should be answered as to how you best identify yourself out of the choices provided.

    • This section gives us a little information about your school. In order to apply for this program, you should only do so with a school Educator (Guidance Counselor, Teacher or other administrator) aware of your application. This individual will be contacted by us if we have any questions or verification needs. We will also let them know the final status of your application. This person will be considered your School Contact. 



    • Though it's not the most important part of your application, we are interested in learning about your standardized test scores.

      If you've taken one or more - and are comfortable sharing for additional consideration - please select one of those tests in the choices below, and then upload the scores in the attachments section.

      It is okay if you haven't taken a test or don't wish to share your current score; please instead select that option.

    • We would love to learn a little more about who you are! In the boxes below, please list a few activities you are or have been involved in. Please share years (or dates) of participation, as well as what the activity entailed. You do not have to complete every box, but we ask that you share at least two.

    • We know essays can sometimes be tough, but please don't let that stop you from applying! We want our Selection Committee to get to know you in a more in-depth way. You don't have to be a great writer to submit an essay - just write with sincerity. The two essays will be reviewed based on multiple factors, including content, passion, grammar, readability, flow and just how well you generally answered the question. One question will be the same for everyone (Personal Statement), and another question will give you a choice in topics to write about (Writing Supplement).

      There is nothing additional to upload - we have provided enough space within the application. We suggest having your essays ready (checked for grammar, word count, and for the content you want to say) in a program like Microsoft Word or an e-mail before you paste into the essay boxes.  

    • Personal Statement: Please share a personal statement about yourself. You can include why you are considering being a healthcare professional one day, and/or why you feel you would be a good fit for this program. You can also opt to share a personal statement that simply speaks to who you are. Please make this statement no less than 250 words and no more than 650 words.

    • Financial Information & Waiver 
    • If accepted, there is a $300 Participant Fee to be in one of our TAP Health Summer Academy programs. This fee is not due until the program begins in June.

      If you are accepted into a program and unable to pay the cost, we offer financial waivers based on need. If you need this waiver, please sign the statement below. Then if accepted, we will apply it to your account automatically.

    • Letter of Recommendation 
    • Please ask an adult you trust to write a letter of recommendation for you. You should give this person as much time as possible to submit their letter by the application deadline. It is best to choose someone familiar with you either as a student, or with regards to work, sport, club, volunteer activity(ies), or other extracurriculars you have been involved in. You can also ask someone else who may have unique insight to why you would be a great fit for the healthcare profession one day.

      • Letters of Recommendation will be submitted separately from your application and should come directly from the recommender.
      • When making your request, they should submit on your behalf at: bit.ly/tap23reference.
      • If for any reason your Recommender changes after you've submitted your application (but before deadline), please let us know the new name to expect on your behalf at: hmckinney@healthcollab.org
    • Lab Jacket 
    • If accepted into a TAP Health program, you will receive a personalized lab jacket/coat. The TAP MD jackets are the longer coats, and tend to run large.

      To help us prepare early, please look at the size guide below and share what size you would need. For example, "L", meaning size large. If you are applying to multiple programs and wear different sizes in each, please indicate.

      Some of our students prefer shortened names or nicknames for their jacket. This isn't required, but we do give you the option. For example, someone might be named Terrence but prefer "Terry". Or someone may be named Catherine but prefer "Kate". Please share how you would like your name to appear.
