Please complete this form to join the ISTT committees. Your application must be approved by your local Affiliated Society before submission. Please upload proof of this approval (a simple email is sufficient) with your submission. Furthermore, you must be a member of the Affiliated Society in good standing to qualify for this application.

To complete this form, you will need:
- CV
- Approval document (email) from your local Society
- Relevant Experience Summary or Statement of Interest
