Declaration

I solemnly DECLARE by virtue of the provisions of the Oaths And Declarations Act 2000, and subject to the penalties provided by that Act for the making of false statements, that

(a) I/We have sought medical advice and I/We have not been advised otherwise by any medical practitioner that I am/We are physically and mentally not fit or not able to take part in the activities and events; and

(b) All information provided by me is true to the best of my knowledge.

Terms & Conditions

· Participants must not be suffering from any medical condition, illness, disease, mental illness or physical / psychological impairment. Players are not advised to participate if they are feeling unwell. In such cases, please inform the event organizer and match officials immediately.

· Migrant Workers’ Centre and Quebec Leisure and its agent reserve the right to change the content of the event with or without prior notice.

· Due to limited places available, only registrations with full payment made are deemed as confirmed on a first-come-first-served basis. Payment received is NON-REFUNDABLE.

· All registered participants for this event will be contacted via email or telephone for further confirmation of their particulars and other event details.

· I declare that I have obtained consent from the Emergency Contact person for Migrant Workers’ Centre’s collection, use, and disclosure of their personal data in case of emergency.

· All participants MUST bring along their NRIC & Employment Pass (if applicable), on the day of the competition for verification by Tournament Officials.

· Managers / Captains shall undertake the responsibility of ensuring the eligibility of their players competing in this Tournament. If participant(s) is / are found ineligible they will be disqualified from the competition with immediate effect.

· Only players’ names that are listed in the Registration form shall be eligible to participate in the Competition. Changes to the line-up will not be accepted, once it has been submitted to the presiding Tournament Official on the actual event day / date.

· In the absence of any teams in any of the competing rounds, a win or default (walk-over) would be awarded to the other opponent by the end of the grace period.

· All protest must be accompanied by a protest form & fee of $100 and form which will be forfeited if protest is invalid. The Tournament Committee will be the Final Decision Maker. It should be made BEFORE or DURING the game, all protests that come AFTER the result has been finalised would not be entertained.

· Submission of the official registration form after the Captain’s Briefing would not be included for the Tournament Draw.

· Players shall comply with the instructions and decision of the event organizers and match officials.

· Any changes / amendments / decisions would be made at the sole discretion of the Organizing Committee.

· All participants are covered under Accidental Medical Expenses Insurance capped @ $1,000 from injuries sustained at the Migrant Workers’ Centre and Quebec Leisure Event.

· All prizes are not exchangeable for cash and not transferrable.

Personal Data

· All registered participants for this event will be contacted via email or telephone for further confirmation of their particulars and other matters pertaining to the event.

· By signing below, you consent to the collection, use and/or disclosure of your personal particulars by Migrant Workers’ Centre and Quebec Leisure for the purposes of organising this event, and/ or uses by Migrant Workers’ Centre and Quebec Leisure for analysis and research purposes pertaining only to matters in relation to such events/ programmes.

· All registered participants will be included into the contact list of Migrant Workers’ Centre and Quebec Leisure, to receive regular updates on future engagement events and programmes organised by Migrant Workers’ Centre and Quebec Leisure, as well as opportunities to contribute, share opinions and/or feedback through any of the contacts provided through any means.

· Please update Migrant Workers’ Centre and Quebec Leisure of any changes in contact details so as to facilitate communication.

· Any photographs, motion pictures, recordings, or any other media records of this event may be used by Migrant Workers’ Centre and Quebec Leisure for any legitimate purpose, including any commercial and marketing uses, and/ or to be uploaded onto Migrant Workers’ Centre and Quebec Leisure’s public sites and channels.

· In the event that registered participants do not wish for his/her photographs and/ or any other media records of him/ her at this event to be used by Organizing Committee, please inform and provide clear instructions to the Organizing Committee in writing.

· In the event that registered participants do not wish to receive regular updates on future engagement events and programmes organised by Migrant Workers’ Centre and Quebec Leisure, as well as opportunities to contribute, share opinions and/ or feedback, please inform and provide clear instructions to Migrant Workers’ Centre and Quebec Leisure in writing, within 14 days of the event. Migrant Workers’ Centre and Quebec Leisure will consequently be unable to inform you of subsequent engagement events.

· Migrant Workers’ Centre’s privacy policy can be found at https://www.mwc.org.sg/wps/portal/mwc/home/privacypolicy , and is incorporated into this Agreement by reference. Please note our privacy policy may be updated from time to time without notice.

I shall not hold the organiser responsible for any damage or loss of property, or any injury, or loss of life, which may be sustained by me during the activity arising from any cause in connection with the activity. I also undertake to indemnify the organiser against any claim by any part in connection with the activity. I HEREBY RELEASE AND DISCHARGE you and or your authorized organizing agent or any other persons authorized by you and or your organizing agent from all claims that I or my personal representative(s) and dependent(s) may have for injury (including injury resulting in loss of life) however caused sustained by me and for loss of or damage (however caused) to my personal belongings suffered at any time while taking part in the activities and events as participant or as visitor, while within the site of the activities and events for any purpose whatsoever.

IN CONSIDERATION OF YOU having at my request agreed to accept participation by my child named below in the activities and events organized by you and or your authorized organizing agent and to grant access to my child to the site of activities and events as participant for the activities and events and or as visitor for other purposes, I below signed being the legal guardian of above registered participant, HEREBY RELEASE AND DISCHARGE you and or your authorized organizing agent or any other persons authorized by you and or your organizing agent from all claims that my said child may have for injury (including injury resulting in loss of life) however caused sustained by my child and for loss of or damage (however caused) to his/her personal belongings suffered at any time while taking part in the activities and events as participant or as visitor, while within the site of the activities and events for any purpose  whatsoever.
