    QUICK GUIDE - BEFORE YOU BEGIN

    Please have the following documents ready before starting this form:

    • PROOF OF IDENTITY (Primary Taxpayer & Spouse/Partner) - Please provide a copy of your Driver's License, FRONT and BACK. If you do not have a Driver's License or State ID Card, then provide a copy of your Passport. We cannot pull your IDs from a prior year, even if not expired. Required annually by the IRS due to increased tax-related identity theft. For Non-Working Spouse and Dependent Children provide Copy of SSN/ITIN/ATIN.
    • PROOF OF RESIDENCY (Children and Minor Dependents) - Please provide a copy of an ID or document that ties your dependent to your home address. This could be a birth certificate (newborns only), medical or school record. You must show that your child and/or minor dependents lived with you during the tax year. Required annually by the IRS.
    • IRS LETTER/TAX NOTICE(s) - If you received the any IRS Letters or Tax Notices during the year, attach the letter.
    • ANSWER QUESTIONS & PROVIDE FOR UPLOAD TAX DOCUMENTS - At the bottom this page Part Four you are to use the "Brouse Files" to include for Upload all your tax documents and forms to submit with this online Tax Organier Checklist. 

    If you are unable to attach your documents or have already sent them, then you are welcome to fill out this form to update your contact, dependent, and payment information without attaching any documents.

    If you have not submitted your documents, we ask you to wait to fill out this form until you have all your documents ready. 

    Please Note: Digital Documents must be Clear and ideally Properly Labeled.

    • Acceptable File Formats: Quality scans as PDFs, Microsoft Word, and Excel documents. You may include a photo image taken from phone of ID's or Documents. It is preferred that you scan as PDF format any documents if they are not already in that format.

  • DOCUMENT UPLOAD: Important: You Must Submit To Us Your Documents:

    If you Recieved any Tax Notice(s) from any Tax Authority during the year kindly upload a copy of them to us as well.
