This form is used to request a rental verification and rental history for a current or former tenant. Please note that a payment of $25 is required at the end of this form, and we will not process any rental verifications without that being paid first. Processing rental verifications takes up our time and resources, and we must be compensated for that. If you are a landlord, apartment complex, mortgage company, or anyone else other than the tenant and you do not want to pay this fee, you can request that the tenant fill out the form instead and include your information to send the verification to.
