    Becoming a Jr Stryke Volunteer with ST4V can be a rewarding experience. JR Strykers have the opportunity to accumulate community service hours through call to actions, events, and other outreach services

  • your signature below states that you understand you or another parent or Guardian must be present with your teen(s) that are participating in the JR Stryke Volunteer Program and that you are responsible for the care and well being of your child/teen. You understand that at no point can you leave the premises without your child/teen unless another Guardian you have appointed stays in your place.

    By signing below you are stating that you have read the code of conduct to your child/teen and they and you understand and agree to adhere to the code, and conduct themselves accordingly.

    Code of Conduct and Ethics

     

    Our StrykeTeam 4 Veterans code of ethics and business conduct outlines our expectations regarding the behavior of our board members and volunteers towards other board members and volunteers, business partners, our Veterans, first responders, and the community.

     

    Our organization stands for the core values of synergy, honesty, integrity, commitment, respect and consistency. As such, we expect all organization members to be directed by the same values in their judgment and behavior.

     

    Open communication and expression should be guided by the desire for a respectful, safe, and collaborative working environment.

     

    Scope

    This policy applies to all organizational members.

     

    Compliance with law

    Volunteers must work to protect the organization and its legal interests by complying with all applicable laws.

     

    Safety in the working environment

    All Volunteers should respect other members, any discriminatory behavior, disrespect or harassment will not be tolerated under any circumstances.

     

    Professionalism

    Volunteers must show integrity and professionalism in every aspect of conduct, including matters involving absenteeism, tardiness, and dress code compliance. Personal appearance should project the organization commitment to professionalism.

     

    Integrity

    All Volunteers should fulfill their duties with integrity and respect toward our partner organizations. Volunteers are discouraged from accepting personal gifts from any other organizations that can benefit them, or partners for the benefit of another party. We are all expected to avoid any personal, financial or other interests that may interfere with our credibility and the welfare of our organization.

     

    Care for the physical environment

    Volunteers should treat private property and their physical environment with respect and care.

     

    Disciplinary Actions

     

    Volunteers who repeatedly or deliberately fail to follow our code of conduct will meet an appropriate disciplinary action. Following a clear warning, Volunteers who persistently show indecorous behavior may face suspension, or immediate removal from our organization.

     

    Legal actions may be taken in cases of theft, embezzlement, corruption, and other unlawful actions

    By signing below you understand that your child/teen may be in media taken at events,call to actions, and other outreach programs of which they are attending.
  • I grant permission to StrykeTeam 4 Veterans & Shield Maiden Productions to use my child/teens image or video publications including videos, email blasts, recruiting brochures, newsletters, and magazines and to use my image in electronic versions of the same publications or on the StrykeTeam website or other electronic forms of media. I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.

    I am the parent or legal guardian of the below named child. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
