In exchange for participation in The French Ring and Training Workshop by Yvan Granados “Bambino” (the Activity) held at the property of Tanya Yost and Duncan Kile (the "Releasees'), I hereby agree as follows:  
 
1.    I, First Name*   Last Name*, (the "Releasor"), understand that dog sports involve inherent risks, including the risk of physical or psychological injury, pain, suffering, illness, disfigurement, and/or death, and I assume all related risks and voluntarily participate in these activities.
 
2.    By participating in this workshop, whether actively or in an audit position, I release the Releasees as well as the workshop organizers, trainers, decoys, and other participants (collectively, the "Released Parties") from liability for any injury or illness that may occur while on the premises, or that may be worsened by my participation.
 
3.    I and anyone, claiming on my behalf release and forever discharge the Releasees and the Released Parties in their individual and/or corporate capacities from causes of action of any nature and kind, known or unknown, which I may have against any of the Released Parties arising out of or relating to any injury, loss, or damage to person and property that may be sustained as a result of participation in this dog sports workshop.
 
4.    I also acknowledge that any dog/s I bring onto the property of the Releasees is up to date on all required vaccinations for Rabies and DHLPPC (canine distemper, hepatitis, leptospirosis, parvovirus, parainfluenza, and coronavirus), free of parasites to include fleas and ticks, and is not displaying any signs of untreated or potentially contagious conditions. I understand that I may be asked to leave if any of the aforementioned requirements are discovered to be untrue. I acknowledge sole responsibility for any and all acts or behavior of my dog.
 
5.    I have carefully read and fully understand all of the provisions of this Release and am freely, knowingly, and voluntarily entering into this release.
