It is essential for the provision of successful healthcare that a sense of mutual trust exists between the employer, the patient and those involved in their care.
All patients have the right to be confident at all times that their dignity and privacy will be respected and that all information associated with them will remain confidential. Except where there exists a "Duty to Act".
The private and commercial sectors are considered to be an essential part of the care given to patients and so are subject to the same confidentiality regulations as NHS employees and are therefore required to sign an undertaking of confidentiality.
I fully understand that in the course of my duties I will have substantial access to confidential information concerning patients, their relatives and Codeblue Medical / ACLS Training Services Ltd Clients.
I undertake to maintain confidentiality in relation to data regarding patients, their relatives and any client for whom Codeblue Medical / ACLS Training Services Ltd provides a service, ensuring that it is processed lawfully; for no purpose other than for which it was obtained; is relevant to that purpose; is retained no longer than is necessary; is processed in accordance with the rights of the subject to access and accuracy; and is protected from accidental loss or damage in accordance with the requirements of the Data Protection Act 2018, General Data Protection Regulations (GDPR) and NHS records management guidance.
I undertake to maintain confidentiality of patient identifiable personal information using a non-identifiable alternative where practical and limiting access on a strictly need to know basis in accordance with my responsibilities with the Caldicott regulations.
I fully understand that under no circumstances must I discuss anything that I have seen, heard, observed or read in regard to patients, I fully understand that under no circumstances must information regarding Codeblue Medical / ACLS Training Services Ltd Clients or Patients treated by the organisations employees or subcontractors be passed to other companies or organisations or be used by any persons except in the normal course of continuity of care.
During my work with Codeblue Medical / ACLS Training Services Ltd, I must not pass on any details of clients and customers to any other Private Ambulance Services or Event Medical Companies or individuals.
Whilst I am working for Codeblue Medical / ACLS Training Services Ltd directly, indirectly or covering a commitment as a Sub-Contracted agency I will not approach Codeblue Medical / ACLS Training Services Ltd clients in any attempt to entice them away from Codeblue Medical / ACLS Training Services Ltd.
In signing this statement of confidentiality I accept all the conditions both stated and implied.
Infringement of this statement will be treated severely under both Codeblue Medical / ACLS Training Services Ltd Disciplinary Policy and/or Legal Representation.
The management team will consider this an act of Gross Misconduct. And will necessitate the immediate cancellation of your contract and the immediate cessation of any work we currently have booked with you, alongside other potential legal actions commensurate with the actions in contravention of this agreement.
