  • *No reservation will be accepted without the return of this signed confirmation. Your signature on this contract constitutes a guarantee for this reservation and compliance with the rules of use.

  • Format: (000) 000-0000.
  • 1-Eligibility: Only residents or employees of Minerva Park can rent.  Applicants must be 21+ years old.

    2-Capacity: Maximum of 168 people per State Fire Codes.

    3-Supervision: Youth events require 1 adult per 10 youths; groups of 30+ MAY require an off-duty officer.

    4-Safety: Use the building safely; staff/police may enter during events in performance of their duties.

    5-Prohibited Items: No firearms, smoking, vaping, tobacco, or alcohol.  Beverages are limited to clear/light-colored liquids.

    6-Parking: Follow all parking signs

    7-Cleanliness: Leave the building clean, return furniture to original places, and dispose of trash in the dumpster.  A Checkout List will be provided for your use.

    8-Decorations: Use removable tabs/putty for wall decorations; no tape, pins, nails or staples.  Use of glitter, craft dyes, paints, etc. is prohibited.

    9-Furniture: Only tables and chairs may be moved; do not drag them across the floor.

    10-Entertainment: Keep noise levels low to avoid disturbing residents.

    11-Damage: Users are responsible for any damage caused.

    12-Fees: $300 rental fee and $300 refundable security deposit due at reservation.  Nonprofits are exempt.  Small groups may qualify for reduced fees at the discretion of the Mayor

    13-Cancellation: Full refund minus $25 for cancellations 7+ days prior; no refund for cancellations within 6 days.

    14-Key:  Return Key Fob after the event; lost key fob incurs a $50 charge.

    15-Indemnification:  Users assume all risks and releas the village from liability for damages or injuries.

    16-Compliance: Follow all laws, ordinances, and regulations

    17-Access: Village officials may inspect the premises at any time

    Indemnification, Wavier and Release. In addition to, and not in limitation of, anything herein or hereafter stated in this Agreement, the Contact Person/Organization listed above (hereinafter referred to as “User”) shall indemnify, hold harmless, and defend the Village of Minerva Park, its officials (elected and appointed), employees, agents, representatives, attorneys, insurers, successors and assigns (hereinafter collectively referred to as “Indemnified Parties” and individually as “Indemnified Party”) harmless from and against, and shall release and waive any Indemnified Party and their agents and employees for, from and against any and all demands, claims, causes of action, fines, penalties, damages, liabilities, judgments, allegations and expenses (including, without limitation, reasonable attorney’s fees) related to, or arising from:

    (1)the use of the Community Building by User or by any person or entity acting on behalf or, together with, or at the direction of User;

    (2)the use of the Community Building by a participant, spectator, invitee, or any other individual or entity on the Community Building premises because of, or related to, User’s use of the Community Building under this Agreement;

    (3)any activity, work, or thing done or permitted by User on or about the Community Building; If any action or proceeding is brought against an Indemnified Party by reason of any of the foregoing (1) through (3), User, upon written notice from such Indemnified Party, shall defend the same at User’s expense, with counsel selected by the Village.

    Acknowledgement of Inherent Risks. User acknowledges that there are inherent risks in certain activities and accordingly, User by commencing and executing this Agreement specifically accepts the Community Building in the condition presented and accepts for itself and its participants any risks associated with said condition and activity. User acknowledges the Village does not make any warranties, express of implied, to the Community Building.

    I have read the rules and regulations for use of the Minerva Park Community Building, and in signing, I agree to abide by these rules and regulations. I agree to accept full responsibility for any damage done to the community building property by those in attendance and to leave the premises clean.

    I have read the foregoing release and fully understand it. The individual signing this Agreement on behalf of an organization acknowledges fully authority to sign on behalf of said organization.
