I, ___________________________________________________, have agreed to participate in Frisco Triathlon Club's events.
In consideration of being permitted to participate in any way in any FTC event, whether sanctioned or unsanctioned ("Activity"), I, for myself, my personal representatives, assigns, heirs, and next of kin:
1) I acknowledge that the Activity may involve strenuous physical activity, including, but not limited to swimming, outdoor cycling, and running. I will follow the rules of the road at all times and understand I am solely responsible for my own safety.
2) I affirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in the Activity.
3) I acknowledge that my registration and subsequent participation in the Activity is completely voluntary.
4) I acknowledge the contagious nature of COVID-19, as well as any subsequent pandemics, and I understand the inherent risk associated with participating in Activities during a pandemic. I acknowledge that FTC has put in preventative measures to reduce the potential exposure to COVID-19 and that FTC is following state guidelines as well as CDC recommendations. That said, I understand the risk of being exposed to and for infected by COVID-19 that may result from the actions, omissions, or negligence of myself or others.
4a) I attest that I am not experiencing any known symptoms of COVID-19 and that I will refrain from participating in any Activity if I experience any known symptoms of COVID-19,
