  • At this time, BOOST a FOSTER FAMILY only accepts referrals for Kinship families experiencing financial hardship or who have other barriers prohibiting them from becoming licensed. Questions? Please contact us here.

  • Boost a Foster Family does not take requests for the following: rent/mortgage assistance, utilities, car repairs, appliances, home remodels, cribs, high chairs, strollers, swings, bicycles, clothing, shoes, diapers, formula, and computers. Please limit your request to items required for Arizona State Licensure.

    Boost a Foster Family does not take requests directly from families. If you are a foster parent needing assistance, please contact your caseworker or licensing worker.

    Please provide the following data on the family in need of assistance. In order to expedite assistance, Boost is requesting identifying information. You must obtain proper permission/releases.

  • Help Needed:

    If requesting a CAR SEAT you MUST add requested information to the COMMENTS section of this table. Without that info, your request may not be filled in a timely manner.

  • CPR/First Aid & Doctor's Physical Requests

    REFERRING WORKER: If requesting CPR/First Aid or Physicals, please alert that family that we will email them a form to be completed within 5 days, and the family must schedule their appointment within 30 days. If the family is not ready, feel free to put in a request at a later day. 

    Many factors are taken into consideration when scheduling caregivers for these services. Please DO NOT tell your client they will get to have classes or a physical done in a certain manner. This will be determined by the Boost Project Manager and the team of service providers and then be offered to the family. Thank you for your cooperation and understanding.

  • Household Repair Requests

    Boost a Foster Family will assist with minor home repairs required for placement of a child or required to pass the Life Safety Inspection. (For Pool Fences please submit a specific Pool Fence Request form).

    Boost will ONLY provide assistance to families for the following when the primary care giver is unable to do the work themselves due to physical or financial limitations. Please explain the family’s situation in the comments section.
