Entry fee: $20

Lunch: $15 - BBQ lunch (pulled pork sandwich, chips, cookie, and drink) is available for purchase for $15 and will be delivered to your fishing
spot starting at 10:00 am. Please preorder using this form to reserve your lunch. Lunches will also be able to order the day of the tournament but may be limited.

Payment: Cash payments will be taken at registration.

Pre-registration: If you pre-register you still need to come by the registration tent at the event and submit payment and pick up team number and flag.

*Each participant will receive 1 raffle ticket for door prizes, additional
tickets available to purchase for $5

*Prizes will be awarded to top 3 largest live length game fish*

Liability Release: I am aware that no ice can be considered to be completely safe. In consideration in participating in the above activity, I hereby take action for myself and waive, release, and discharge any and all claims for damages for death, personal injury, property damage, theft or actions of any kind which I may have, or which may hereafter accrue to me, as a result of participation of said activity. This release is intended to discharge in advance Alpine Outfitters, their officers, agents, instructors and volunteers; the activity sponsors officers, directors, volunteers and instructors from liability, even though that liability may arise out of negligence on the part of the persons of entities mentioned above. Recreational activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks.

COVID Release: By attending this event I am attesting that I do not have any covid symptoms and I am assuming all personal risk and cannot hold any entity responsible.

I agree to follow all Colorado fishing regulations and have in possession a CO fishing license (Lake San Cristobal has a 4 trout limit).
