This is a contract, please read before signing:
In consideration of Christina Dadamo Dance Academy allowing me or a child for whom am parent or guardian to participate in activities including but not limited to dance, cheer and gymnastic, I agree to forever release, indemnify and discharge Christina Dadamo Dance Academy and all their affiliates, agents, officers, directors on behalf of myself, my spouse, my children, my parents, my guardians. and my heirs, assigns, personal representative and estate. and any and all other persons and entities who could any way represent me, the Minor Child or act on our representative behalfs.
1. Thereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Christina Dadamo Dance Academy from any and all claims, demands, or causes of action which are in any way connected with my participation in Christina Dadamo Dance Academy activities.
2. Release and hold harmless Christina Dadamo Dance Academy and its agents from any and all financial obligations or other liabilities that may be incurred as well as any damage or injury to students or the person or property of others that students may cause while participating in this program. Students and parents agree to indemnify and hold Christina Dadamo Dance Academy harmless against any financial liabilities.
3. Students are expected to behave properly and exercise caution and common sense.
4. Should a student become ill or incapacitated the students and parent/guardian grant the school or any of its agents full authority to take whatever action is believed necessary under the circumstances regarding they student's health and safety. including medical treatment at the expense of the parent/guardian and/or the student. The school will be released from any liability for related decisions or actions which may be taken on the students behalf,or for the quality and timeless of any medical care, or for the expenses so incurred. I certify that have adequate insurance to cover any injury or damage I may cause or suffer while participating or else I agree to bear the costs of such injury or damage myself. further certify that am willing to assume the risk of any medical or physical condition may have. In consideration of not being required to sign a fresh copy of this release before each visit, - further agree that this release shall apply to all future visits by me and by the minor participant until he/she is 18 years old.
5. Understand it is my responsibility to inform the Christina Dadamo Dance Academy of any allergies my child has.
6. Christina Dadamo Dance Academy has my permission to photograph, record and videotape me/my child for promotional use, used only for Christina Dadamo Dance Academy, with no compensation due me or my child.
7. This agreement shall be binding upon the parties and their heirs, successors, administrators, executors and assigns.
8. It is the parent or guardian responsibility to notify the school if the child has any allergies or medical conditions.
I am aware that these rules apply if we are performing or practicing at any venue with Christina Dadamo Dance Academy. including, but not limited to practicing at an outdoor space or competition. I have read and understand each and every term and condition as set forth herein and shall equally apply to the below I represent that am the parent/guardian of the Minor Child.
