* Before membership applications can be accepted, the membership application form must be completed, signed, and returned to ADP along with all required supporting documents.

* Upon approval of your application, you will receive a confirmation letter from ADP.

To apply for membership, please fill out the form. All necessary documents must be submitted in English. Make sure all information is accurate. Please provide ADP with your mailing address so that certificates and other relevant documents can be sent to you in the future.
