    PLEASE NOTE: Outside Perspectives works with every family to make expeditions accessible. Outside Perspectives will not exclude a participant based on mental health condition(s) or behavioral history unless the youth’s safety and well-being, or the safety of others, cannot be reasonably maintained or managed in all conditions and circumstances. The following information is primarily used to support the participant while on the expedition. Removing barriers and getting youth outdoors is part of Outside Perspectives mission.

  • INFORMATION & CONSENTS

  • Short Course Information and Consent to Participate

    Outside Perspectives conducts physically active courses, which may include running and participating in physical games and activities. Unless otherwise specified by your organization or school, programs occur outside and potentially during inclement weather. There are certain inherent risks to be assumed when participating in activities of a physical nature, and the participant may risk personal injury.

    Outside Perspectives instructors and staff will inform participants of specific safety rules and teach necessary skills before each activity.

    Due to the nature of the course, the program goals, and the possibility of damage, participants should not bring electronic devices to the program, including cell phones. Use of electronic devices will not be allowed during the program unless an exception is communicated to instructors beforehand. Outside Perspectives will not take responsibility for loss or damage to any electronic devices. Upon request, participants can be given a safe place to store devices for the duration of the program. Instructors will carry cell phones for emergency communication.

    Outside Perspectives' course participants are expected to treat each other with respect and kindness. Intentional group dynamic development and individual support and coaching will provide opportunities to overcome any interpersonal challenges that may arise.

    Outside Perspectives is a challenge-by-choice program, which means participants ultimately have the free will to decide to participate in the program and its activities. However, once the participant has committed to coming on the course, they are expected to stay until the end and participate as best as they can, except under unforeseen and unexpected circumstances.

  • One-Day Back-country or overnight Information and Consent to Participate

    Outside Perspectives conducts physically active courses, which may include running, playing games, hiking with a backpack, rock climbing, canoeing, kayaking, car camping, and participating in activities during inclement weather. There are certain inherent risks to be assumed when participating in activities of a physical nature, and the participant may risk personal injury. While the course can be challenging, it is expected that any person with typical physical and mental abilities can complete the program successfully.

    It is very important that participants make known any and all relevant physical and mental health information, which should include medications and dietary needs, in the medical forms. Omitting this information may affect the safety of the individual and others.

    Outside Perspectives instructors and staff will inform participants of specific safety rules and teach necessary skills before each activity. Participants must be aware of and agree to the following rules while on the course.

    1. Stay with the group at all times, do not attempt to leave the course on your own.
    2. Alcohol, recreational drugs, and cigarettes are prohibited.
    3. Personal knives are not allowed on the course.
    4. There are no electronic devices on course.
    5. Prescription medications must be taken in accordance with Doctor’s Orders.
    6. Shoes must be worn at all times, except when instructors deem it safe to be barefooted.
    7. Listen to and follow all procedures for specific activities.
    8. Show respect for all Outside Perspectives participants and staff.
    9. Show respect for the environment, public property, and private property.
    10. Show respect for yourself -- always be willing to try.

  • Expedition Information and Consent to Participate

    Outside Perspectives conducts physically demanding courses which MAY include running, playing games, hiking with a backpack, rock climbing, canoeing, kayaking, backcountry camping, and participating in activities during inclement weather. There are certain inherent risks to be assumed when participating in activities of a physical nature, and the participant may risk personal injury. The participant must be emotionally as well as physically prepared for the rigorous demands of the experience. Outside Perspectives instructors and staff will inform participants of specific safety protocols and will coach participants before each activity. While the course can be challenging, it is expected that any person with typical physical and mental abilities can complete the program successfully.

    Expedition participants take part in backcountry expeditions that include camping and hiking with a backpack and may include canoeing and rock climbing. Participants sleep in tarps or tents inside sleeping bags (not cabins) for the entire course and may carry thirty to fifty-pound backpacks an average of five miles per day and for extended periods of time. Expeditions occur in remote areas and in all types of weather, including wind, rain, cold, heat, and electrical storms. While many precautions are taken to avoid contact with animals, insects and bacteria, environmental hazards include potential exposure to diseases such as tick or mosquito-borne illnesses (ie: Lyme Disease) and Giardia.

    Participants must be aware of and agree to the general policies and rules listed in the Participant Agreement. The Participant Agreement will be shared with participants prior to the expedition and may also be viewed on the Outside Perspectives website. Behavioral policy violations require intervention from program staff. Intervention strategies will be utilized in the field on-course whenever possible. Removal from the expedition may result if a participant demonstrates risk to self or others’ physical and/or emotional safety.

    Due to the physical nature of the activities and the remote locations, it is very important that participants inform Outside Perspectives of any and all relevant physical and mental health information, including medications and dietary needs, within the required medical forms. Omitting this information may affect participant safety and the safety of others. Outside Perspectives will not exclude a participant from an expedition for a health or medical reason unless participation provides an unmanageable risk or burden to instructors, the individual, or to the group, as determined by Outside Perspectives. Removing barriers to the outdoors is part of the organization’s mission.

    Outside Perspectives provides ample and nutritious meals prepared by the expedition participants and staff. All drinking water from natural sources are purified by approved methods such as boiling, filtration, or by chemical (i.e. Chlorine Dioxide) treatment prior to consumption. Personal hygiene and self-care methods are limited in a wilderness setting and include cold water clean up. Toilet facilities are latrines and outhouses.

    Due to the goals and nature of the expedition (and the potential risk of damage to devices) electronics are not allowed on expedition; this includes cell phones. Outside Perspectives’ staff will carry cell phones for emergencies or other unforeseen circumstances that require communication.

  • Adult Leader/Coach Short Course Information and Consent to Participate

    Thank you for volunteering to come on this one-day short course with the youth you work with. Having you be a part of the one-day short course will help build cohesiveness amongst the group and is extremely valuable to help continue the learning process after the course is over.

    The one-day course you are about to help lead is a physically active course, which may include running, playing games, and participating in activities during inclement weather. There are certain inherent risks to be assumed when participating in activities of a physical nature, and by taking part in this course, you may risk personal injury.

    Please make known any relevant health information you may have to instructors before the course, which should include dietary needs, in the medical section of this form.

    Outside Perspectives instructors and staff will be in charge of teaching all necessary skills as well as overseeing the safety of the group. As an adult leader, you must be prepared to clearly communicate your needs to the instructors, help supervise the group, and encourage positivity during the course.

    Due to the distractions they cause, and the risk of damage to devices, electronics are not allowed on courses by any participant. This includes cell phones. There is an expectation that your phone will be turned off or to silent during the course and that you will not be seen using your phone in front of the group. Please communicate with instructors about exceptions that may be needed while on course. Outside Perspectives' staff will carry cell phones for emergencies or other unforeseen circumstances that require communication.

    Outside Perspectives is a challenge by choice program, which means participants ultimately have the free will to decide to participate in the program and its activities. The participation of Adult Leaders in specific activities can be powerful for participants and help them to be engaged more fully. Participation in activities by Adult Leaders is not required, but we strongly recommend engagement through either participation or active support.

    Once a participant has committed to coming on the course, they are expected to stay until the end and participate as best as they are able, except under unforeseen and unexpected circumstances. We ask that you help support participants participating and completing the day.

  • Adult Leader One-Day Activity/Overnight Camping Information and Consent

  • PLEASE NOTE: Instructors are certified as Wilderness First Responders. Conservative judgment is used with the treatment of any participant's sickness or injury. Administration of internal and external over-the-counter medications occurs only when other means to manage the situation have failed to produce the intended results. What we carry in our first aid kit... Topical: Alcohol Pads, Hydrogen Peroxide, Anti-itch Cream, Poison Ivy Cream (non-steroid), Insect Repellent, Sunscreen, First Aid Ointment. Internal: Antacid tablets, Throat Lozenges, Ibuprofen (Motrin), Acetaminophen (Tylenol), Diphenhydramine (Benadryl).

  • PLEASE NOTE: Prescription Medications cannot be administered without 1) a physical copy of a Doctor’s Order 2) original prescription container 3) the original prescription label on the original container. Both doctor’s orders and prescription container(s) must include the patient’s name, medication name, dosage, time of administration, and how it should be administered.
