WEEKLY GROUP AGREEMENT

Fee for group is $___________ per session payable at each session.

This is an open group that meets once a week. There is a 6 sessions commitment to the group once you join. The group doesn’t end after 6 sessions, but your commitment does. The group goes on indefinitely and each member is encouraged to continue in the group, as long as you benefiting from the group.

Each group member is allowed one free miss every 6 sessions.

All members agree to keep who is in the group, and what is shared in the group confidential.

All members agree to keep a sense of good will towards each other at all times, both in and out of group.

Each group member is equal and brings a unique piece to the group. There is no requirement to be in this group, other than a desire to improve yourself. This is not a 12-step group.

Upon exiting the group, member will show up in person and announce they are leaving the group. Member will show up one more time to allow group to process their departure.

Group members agree to not pair off sexually or romantically while they are members of the group.

Group members agree to not come to group under the influence of drugs or alcohol.
