Please enter all information requested below, including Family Service Agreement information, emergency contacts, and medical information. If you have any changes in address or parent/guardian contact information (email and/or phone number), please make the changes on the re-registration form that was sent home with all students on January 25 and return it to your child's teacher by February 15.

Additional documents for registration must be submitted to your child's current homeroom teacher by February 15, 2023. All of the forms listed below were emailed to families, and can also be found on the school website (www.sttaob.com).

- Updated medical and vaccination records (all students entering Kdg, 3, & 6)

In order to receive the Catholic family tuition discount, you must be registered at a Catholic Parish, and have an updated Parish Verification Form on file. Please be sure that you have submitted all applicable Sacramental Certificates, including Baptismal, First Reconciliation, First Communion, and Confirmation to to the school office.

Family Service Initiative

Tuition for students in grades Kindergarten through 8 includes a discount for parents/guardians who participate in our Service Initiative. If families choose not to volunteer, an additional $700 per year will be charged to cover lost revenue. Each family is responsible for working 7 bingos per year, or volunteering for another service position for an equivalent of 28 hours (based on availability).

There are people registered in the Parish who can be privately contracted to work bingos for a $50 fee per bingo. After you receive your schedule, contact the Bingo Coordinator Jennifer Bachonski in the Parish Office at 732-251-4000 X 8210 if you are interested in contacting a replacement.

Vaccination Requirements
Immunizations are required in the Diocese of Metuchen and students cannot be enrolled in a school in the Diocese unless immunized. NJAC 8:57-4.4 (b) allows religious and affiliated schools to grant or withhold enrollment to non-immunized students without challenge by a secular health authority.*
