US Military Discount Form

Beginning in 2023, USA BMX began offering a US Military Discount for memberships. As a reminder, the discount is only available to the USA BMX member that served and the membership must have been purchased ONLINE. Please fill out this form and submit the required information & verification. Our team will verify the information, then credit your card used for the $10 military discount.
Proof of Military Service

The following forms of documentation are acceptable to upload as Proof of Military Service:

Service Members
- Leave & Earnings Statement (LES)
- ORB/ERB (US Army)
- Reserve Activation Orders
- Enlistment Contract
Veterans
- DD214 form
- NGB form 22
- Reserve Separation Orders
- Veterans Health Identification Card (VHIC), or Veteran Identification Card (VIC)
- Driver’s License or DMV-Issued State ID with VETERAN endorsement
Retirees
- DD214 form (must show 20 years of service or the reason for discharge was retirement)
- NGB form 22 (must show 20 years of service or the reason for discharge was retirement)
- Retiree Account Statement (eRAS)
