    Accessibility Notes:

The assessment is best taken on a laptop, tablet or desktop.

If using a tablet or mobile phone, it's best to use in landscape mode.

Some questions within this assessment use 'widgets', such as sliding bars or arrows you will have to move to a chosen position. If you have any particular accessibility needs, or device difficulties, which mean you will not be able to take or complete the assessment, please use the website chat function to talk to us and we can enable the assessment another way for you. If you are part way through the assessment and then find this problem, do not worry as we can help save your progress from being lost.

You can save your progress at any time, and complete later. You'll be emailed with a link to your assessment, so you can resume later.

*Currently the save function has an issue - This only affects answers you select using sliders and where questions have multiple dropdown boxes. We apologise for this and are chasing up a solution. If loading the form to resume from saved, review your answers at the end and check they are correct before you press submit.*

Note on 'required' questions: There are quite a few 'required' questions that you cannot leave blank. They have a red star* by the question text, so that you can identify them.

Privacy note: We will not disclose your assessment and personal data to others without your express permission. This includes insurance companies and medical providers. Please see our privacy policy on the website, for full info.

Equipment to measure your physical attributes,

    (where relevant):

It will help if you have either a tape measure, or a piece of string, that is long enough to go completely around your body at the height of your belly button and your hips.

It will also help if you know, or can measure, your weight (in kg) and height (in cm).

If you don't have any of these, please continue anyway, as there is another way of making the assessments. Depending on the priorities or goals you select during the assessment, these may not be required anyway. If needed, you can follow up with that information later anyway.
