To be approved for net terms with American Tees, a representaive from your oraganization must check all of the following boxes and sign at the bottom.
Must submit an approved purchase order number with each order submitted to begin production.Each invoice has a term of 30 days from the date the order has been completed/invoiced.An Email will be sent to the accounts payable contact(s) (listed above) when the invoice requires payment. It is the responsibility of the A.P. Contact(s) to pay from that notification email.Payment is Requires via cash, check or online payment within 30 days of the invoice date.Late payment fee of 3% per month will be added to the invoice after 30 days from invoice date unless an extension has been requested via email and approved with American Tees.Should the information change on this form, It is the responsibility of the undersigned to update American Tees in writing within 30 days.

Please read through the following options and initial one ordering option that you would like American Tess to adhere to when accepting orders from your organization.
I authorize anyone in our organization to place a quote only for approval to the following email addresses. These quotes will go to the following email(s) and will await approval and purchase order number. Please list those that are allowed to approve quotes for your organization below by listing their name, phone and email address below:
Person(s) who are allowed to approve submitted quotes for orders on terms.
I only authorize the following people to place orders under our organization for terms payments with an approved purchase order number. Any other person(s) must pay out of pocket for their personal orders and will not be allowed to place any orders on terms.
Approved person(s) who are allowed to submit quotes and orders for terms payments.
I authorize anyone in our organization to place an order for terms payment as long as they have an approved purchase order number from our organization.
