Photo Release Consent 照片使用同意表
From time to time, we take pictures of your child during our activities. There may be times when we would like to post pictures of your child or share class work that your child has done on the bulletin board, newsletters or on church’s Sunday school website. (Our Sunday School website is on the church website ceact.ca) We would only share your child’s picture or works if you give us permission.

有時候我們會在活動中拍攝您的孩子及他們在課堂上做的作業。有時我們希望可以張貼在教會的布告欄上，或是分享在通訊郵件或教會主日學網站上。（我們的主日學網站在教會網站ceact.ca上）只有在您同意的情況下，我們才會分享您孩子的照片或作品。

After Class Release/ Pick-up Permission 課後接送許可
I will pick up my child after Children's Sunday School is over every Sunday. 每個星期日主日學結束後我會來接他。I give permission to (please specify below) pick up my child from Sunday School after class is over. 我允許（請在下方欄位填寫姓名）在星期日主日學後接我的孩子離開。I permit my child to leave the classroom on his/her own after the Children’s Sunday School is over. I will take all the responsibilities of my child’s safety and well being once he/she leaves the classroom. 我允許我的孩子在星期日主日學結束後，獨自離開教室。一旦孩子離開教室，我將為他的安全和健康負責。
