HandbookInitial* Martial Arts student has received copy of ETO Student Handbook and agrees to follow guidelines.DuesInitial* Only the 1st month of membership may be pro-rated based on the initial date of registration.Initial* ETO fees are based on a 4 week month.Initial* Only 1 complimentary introductory workout without further obligation may be granted.Initial* Monthly fees are due by the 10th of each month. A $10.00 late fee will be applied after the 12th of each month. A $15.00 late fee will be applied after the 20th of each month. Consistent late payments may require prepayment for future instruction.Initial* Testing fees are due no later than the Thursday before test date. Initial* A fee payment lapse of 1+ months will require re-registration to re-activate ETO membership. Current ETO fee amounts will apply for re-registration.Initial* If there is no lapse/delay in monthly fee payment, ETO members will be allowed to remain at initial fees. In the event a family member joins/exits the program, new fees will be determined according to the new fee structure for remaining family members. (EXCEPTION: For a long member family who has 1 member exit the program, the initial rate for 3 members may be less than new rate for 2 members – in this case the initial rate for entire family will the honored.)LiabilityInitial* Participant acknowledges that ETO Martial & Fitness involves highly physical activities which involves cardiovascular & muscular exertion as well as physical contact.Initial* Participant releases ETO Inc., its owners, operators, and instructors from any liability or injury which may arise from any workout held at ETO located at 2689 South Main Street, Kennesaw GA.Initial* Participant releases ETO Inc., its owners, operators, and instructors from any liability or injury which may arise from any outside event/activity (including demos & tournaments) that ETO is involved with.Initial* Participant assumes responsibility to inform ETO instructor and staff of any physical/emotional condition and/or Doctor’s recommendations that may require any modifications to exercise or workouts.EquipmentInitial* Sparring equipment is mandatory and must be purchased within the 1st 2 weeks of registration date.Initial* Mandatory equipment includes mouthpiece, headgear, hand and foot pads to protect self as well as class mates. Students will not be allowed to spar without pads.Initial* Highly recommended pieces of equipment to provide additional protection include pads for shins, forearms, groin, and chest. If student chooses not to wear recommended equipment, he/she acknowledges and assumes all liability for injury resulting from the decision.
