Thank you for choosing ATC Academy.  Step one secured your entrance number.  This is the second step in the registration process.  Please take your time to fill out this form accurately. Complete answers, using upper and lower case letters, are appreciated. If you are registering a sibling, please complete the appropriate registration form for each child.

Please note any special factors we should consider or be aware of when placing your child in a class (example: emotional or educational needs, parent work needs, other siblings attending ATC, IEP, etc.).

Kindergarten Applicants are asked to provide a Preschool Teacher Assessment. Please print this form, sign Parental Release, and submit to your child’s current preschool teacher.  This form may also be picked up in the office.

I attest that the information on this Registration Form is true and accurate.  I understand that the non-refundable registration fee of $50 must be submitted for the registration process to be complete.  ATC will send a letter regarding placement within three weeks of registering.  To confirm acceptance of placement, the first tuition installment is due within two weeks of receiving placement letter.
