  • I have read all of the above questions. I certify that the information furnished in answer to these questions is correct and complete to the best of my knowledge and belief, and I understand that it is of great importance in the consideration of my eligibility for apprenticeship. I make this statement to the Joint Apprenticeship Training Committee with the understanding that it will be used by the Committee in carrying out its duty in selecting Apprentice applicants. I further understand that any false statement or omission of material fact may be sufficient cause for rejection of my application, or dismissal after my indenture or employment.

  • COMPLAINT PROCEDURE

    Title 29, 30, CFR Section 30.11 (a) and (b)

    Any apprentice or applicant for apprenticeship who believes that he or she has been discriminated against on the basis of race, color, religion, national origin, or sex, with regard to apprenticeship, or that the equal opportunity standards with respect to his or her selection have not been followed in the operation of an apprenticeship program, may personally or through an authorized representative, file a complaint with the Department of Labor. The complaint must be filed no later than 180 days from the date of the alleged discrimination or specified failure to follow the equal opportunity standards. The complaint shall be in writing and shall be signed by the complainant. It must include the name, address and telephone number of the person allegedly discriminated against, the program sponsored involved, and a brief description of the circumstances of the failure to apply the equal opportunity standards.
