I read, comprehended, and honestly answered all of the questions on this questionnaire. I acknowledge that this is a complete disclosure and replaces all prior oral or written disclosures. I am aware that withholding information or spreading false information may cause treatment-related contraindications and/or skin discomfort. I acknowledge that the practitioner shall not be liable should I fail to inform the practitioner of any changes to my medical profile and I hereby commit to keep the practitioner informed of any such changes. I am also aware that any inappropriate or sexually explicit comments or approaches I make will result in the session ending immediately, and I will be held responsible for paying the scheduled appointment. I acknowledge that the treatments I receive here are voluntary, and I waive all claims against O’Luxe Body and assume full responsibility thereof. I give permission to my Esthetician to perform the waxing procedure we have discussed and will hold her harmless from any liability that may result from this treatment. I agree to adhere to all safety post care including: no peels, tanning, or wet room services; no swimming/spas/hot tubs for 72 hours after waxing; and all home skin care protocols as recommended by my service provider. I understand that the service will be provided by an esthetics student and that the student is not yet as skilled, knowledgeable, or trained in all of the procedures that a fully licensed esthetician would be expected to be aware of.
