TENANT MOVE-OUT ACKNOWLEDGMENT. By submitting this form, Tenant acknowledges, understands, and agrees to the following terms and conditions regarding the termination of Tenant’s lease and surrender of the premises: Termination of Lease and Surrender of Premises. Tenant’s lease shall terminate on the move-out date specified in this notice (“Lease Termination Date”). On or before the Termination Date, Tenant shall fully vacate the premises, remove all personal property and belongings, clean the dwelling unit (including the storage shed, if applicable), mow and maintain the yard, and leave the property in a clean, orderly, and rentable condition suitable for occupancy by a subsequent tenant. Required Notice of Termination. Tenant acknowledges that, pursuant to the Lease Agreement, a minimum of thirty (30) days’ written notice is required to terminate tenancy. If Tenant provides less than the required thirty (30) days’ notice, Tenant shall remain responsible for payment of the following month’s rent and any other amounts due under the Lease Agreement. Tenant further understands that failure to provide proper notice may affect the return of the security deposit and may result in legal action to recover amounts owed. Final Inspection. Tenant understands that Tenant may request a final walk-through inspection with Landlord or Landlord’s designated representative. Any request for a final inspection must be made no less than seventy-two (72) hours prior to the intended move-out date and time. Notification of Move-Out. Tenant agrees to notify Landlord of the actual date and time the premises are vacated so that the property may be secured and the locks may be changed promptly following surrender of possession. Thermostat Setting Upon Vacating. Prior to vacating the premises, Tenant shall leave the thermostat set at either: Heat: 50°F; orAir Conditioning: 80°F, as appropriate for the season. If Tenant is uncertain which setting is appropriate, Tenant shall leave the thermostat set to Heat at 50°F. Return of Keys. Upon vacating the premises, Tenant shall return all keys, access devices, and other property belonging to Landlord. Keys may be left on the kitchen counter provided Tenant locks all exterior doors and ensures the premises are secured upon departure. Utility Charges. Tenant acknowledges that electric utility charges are billed one month in arrears. Tenant shall remain responsible for all electricity charges incurred through the date possession of the premises is surrendered, as determined by the utility meter reading on the move-out date. Security Deposit. Tenant acknowledges that, in accordance with applicable Louisiana law, any refundable portion of the security deposit, together with an itemized statement of any lawful deductions, will be mailed to the forwarding address provided by Tenant within thirty (30) days after termination of tenancy and surrender of the premises. Property Showings. Tenant acknowledges and agrees that, pursuant to the Lease Agreement, Landlord may show the premises to prospective tenants, purchasers, lenders, contractors, or other authorized persons during the final thirty (30) days of Tenant’s tenancy upon reasonable notice as required by law and the Lease Agreement. Acknowledgment and Trespass Notice. By submitting this form, Tenant acknowledges that Tenant has read, understands, and agrees to the terms set forth herein. Tenant further acknowledges that once possession of the premises has been surrendered and the tenancy has terminated, any re-entry onto the property without Landlord’s prior authorization may be deemed unauthorized entry and may constitute trespassing under applicable law.
