By submitting this form, I understand that my lease will end on the lease termination date chosen above. I understand that I must be completely moved out, with the property cleaned and ready for the next tenant by the lease termination date. I understand that according to my original signed lease, I must give a full 30-day notice. If I give less than a full 30-day notice, I understand that it WILL impact my security deposit. I understand that I am encouraged to request a final walk-through with the landlord or his designee (All request should be scheduled 72 hours in advance). I understand that I am to turn in all the original keys, which may be left on the kitchen counter (Please lock the door and pull it to upon exiting). I understand that I am to inform the landlord of my actual move out day so that he can come by to secure the property and replace the entry locks. I understand that upon leaving, I am to have the HEAT set to 50 degrees or AIR set to 80 degrees, whichever is appropriate for the season (If in doubt, choose heat). I understand that according to Louisiana's landlord / tenant laws, I will receive the remaining security deposit, along with an itemized deduction sheet mailed to the forwarding address listed above within 30-days of me terminating my lease.
