Welcome to the 2023 Annual Scholarship Application. Please review the scholarship guidelines to ensure you have everything you need to start the application process. The application is estimated to take at least one hour. You will not be able to modify your application once submitted, but you will be able to save your work by simply returning to the form. Submit the names of two references. One must be from a current teacher or counselor from your high school, college or university. The other must be from a non-relative who is familiar with your academic achievements, community services and/or professional performances. The reference must complete the online reference form by the application deadline. The letter should be limited to one page, single spaced in length.

Transcripts should be submitted directly from the registrar to scholarship@dstphoenixalumnae.org. If official transcripts can only be mailed, please mail the sealed transcript to the following address:

All Transcripts both electronic or mailed must be received by the scholarship deadline.

Submit a personal statement. The personal statement can be in a written format (pdf format) of no more than 500 words, a 3-minute video  or a web-based presentation (slideshow, blog, webpage, etc).

Your statement should specifically discuss the following:
a.Your current career goals and what steps you are planning to take to achieve them.

b. Your commitment to community service - such activities may include organizing and/or providing leadership of groups for the benefit of the community, planning or involvement in programs to address community issues or needs.

• I have met ALL eligibility requirements as outlined and acknowledge that
  misrepresentation of the information presented may be grounds to rescind the
  scholarship award. I also agree to inform the scholarship committee of any significant changes to that information.
• I am willing to participate in a personal interview and forward any additional
  information, if deemed necessary.
• I agree to accept the decision of the Scholarship Committee of Delta Sigma Theta Sorority, Inc., Phoenix Metropolitan Alumnae Chapter.
• I will use the proceeds of any scholarship received for the payment of tuition, required fees, room and board, and/or required materials only.
• If I am awarded a scholarship, I will provide satisfactory evidence, as required by Delta Sigma Theta Sorority, Inc. Phoenix Metropolitan Alumnae Chapter, of my full-time enrollment during the period(s) for which the scholarship is awarded.
• I am willing to participate in the Delta Sigma Theta Sorority, Inc., Phoenix Metropolitan Alumnae Chapter Scholarship Awards program.
• I understand that submission of this application constitutes permission to use my name and/or photograph for promotional purposes in all Delta Sigma Theta Sorority, Inc. publications.
• I agree to the release of official transcripts of my grades to Delta Sigma Theta Sorority, Inc. Phoenix Metropolitan Alumnae Chapter.
