  •  - MetroPlex Staffing requires all 8 questions, below, to be answered prior to employment.

  • Question #1:

    I understand time sheets are due Mondays by 10:00 any time sheet that arrives after 10:00 am will NOT be processed until next weeks payroll, no exceptions! This is your responsibility! The time sheet is considered invalid without the approval signature from the property manager. Time sheets without the approval signature will not be processed, until signed by the property manager. All over-time will not be paid without the manager's signature authorizing overtime. All temps are responsible to reach out to property managers and MetroPlex to make sure their timesheets were sent on time, signed and hours provided are correct.  I understand this is soley my responsibility and not MetroPlex Staffings.

  • Question #2:

    I understand MetroPlex Staffing offers direct deposit, should I decide to have my check mailed I am releasing MetroPlex staffing of any and all responsibilities regarding my check. I also understand that MetroPlex staffing will have to put a stop payment on my check in which I will be charged a $30.00 stop payment fee and will have to wait until the following week when payroll is processed to receive a new check

  • Question #3:

    When accepting an assignment with MetroPlex Staffing. I will complete the assignment until it has ended. If I fail to call and /or walk off an
    assignment(regardless of my reason why) all wages due to me from MetroPlex Staffing will be dropped to minimum wage and I will be classified as a "DO NOT USE" employee.

  • Question #4:

    Associates must report immediately to MetroPlex staffing when assignment ends. Failure to do so may affect unemployment benefits. Associates are also asked to call in availability every Monday and Thursday. Failure to do so may affect unemployment benefits

  • Question #5:

    Drugs and alcohol in the work place will not be tolerated. Any employee under the influence will be immediately terminated and placed on our Do Not Use list.

  • Question #6:

    Any injury in the workplace is to be reported immediately, within 24 hours.  An agent from MetroPlex Staffing will instruct you of how to properly seek care upon reporting the injury within 24 hours. If you fail to report the injury immediately, your claim is invalid. MetroPlex Staffing will assist you on a facility to immediately have testing for drugs and alcohol prior to receiving care for such injury. Failing to report to a facility an Agent from MetroPlex Staffing instructs you to go to, will result in an invalid claim.

  • Question #7:

    By agreeing to our terms, I certify that all of the information contained in this application is true and complete. I understand that any misrepresentations or false information that I provide, either in this employment application or at any other time during the application and hiring process, may result in the disqualification of my application for employment or, if I am hired, in the immediate termination of employment at any point in the future.

  • Question #8:

    If I am hurt on the job and live in service I understand that I must choose a treating doctor from the list of doctors in the network. Or, may ask my HMO primary care physician to agree to serve as my treating doctor. If I select my HMO primary care physician as my treating doctor,  I will call Texas Mutual Insurance Company at (844) 867-2338 to notify them of my choice.I must go to my treating doctor for all health care for my injury. If I need a specialist, my treating doctor will refer me to a specialist. If I need emergency care, I may go anywhere. Texas Mutual will pay the treating doctor and other network providers for the treatment for my compensable injury. I may have to pay the bill if I get health care for someone other than network doctor without prior network approval. Knowingly making a false workman's compensation claim may lead to a criminal investigation that could result in criminal penalties such as fines and imprisonment
