Early bird fee of $50 will be added onto total services if the day is scheduled 7 am and earlier or after 12 pm and later. I require a headcount of at least 3 to travel to on site locations. If headcount is not met, $50 will be added onto total.

SERVICE - Subject to terms and conditions in this agreement, Alexis Palos agrees to provide makeup for every person accounted for in this agreement on the day of the event. *Please allow at least 60 minutes per person when planning the itinerary. PAYMENT & TERMS - An initial, non-refundable, security fee of $50 is due upon signing this agreement and the remainder of the total sum of services is due the day of the event, upon completion of services. This fee is only to secure and hold your date, it does not go towards the end total. If the agreement is not returned and down payment is not received within 7 days after trial, the wedding date listed will not be secured. 30 days before wedding date, contract and forms will be sent again for final confirmation. Once contract and form are both filled out and signed with timeframe and headcount, both are final. If a member of the group no longer wants their makeup done or group is causing the scheduled timeframe to run behind, total headcount in contract will still need paid for. If additional members are added on in the last minute timeframe and accepted by artist, $50/hr rate will also be added on for convenience. For payments, half of total headcount will be accepted up to 2 months prior and the remaining total up to a month before the wedding. For day of payments, payments need to be paid prior to service being completed. Pre-Wedding Skin Prep - No facial waxing 48 hours before the event. Chemical Peels/Microdermabrasion cannot be done closer than 40-60 days before the event. If said treatments are performed, bride or makeup participants may not be eligible to receive makeup services. Day of, please have a clean and moisturized face. It is recommended to receive a dermaplane service the week of, this will make makeup application be much more flawless and smooth. If a spray tan is a interested service, this needs done after a dermaplane and wax by minimum of 48 hours. GENERAL - Brides are recommended to get a makeup trial no later than 4 weeks before the event. MISCELLANEOUS - in the event that any equipment owned by the esthetician is broken by the client or party of the client, the client is responsible for the cost to replace, repair, or replace the broken item. This includes but is not limited to airbrush compressor, airbrush gun, airbrush hose, eyeshadow palettes, makeup case, personal items such as cell phone or camera, and any items brought to the event to be used for the event. Release and Waiver - Client further agrees to release and hold harmless Alexis Palos from any and all allergic reactions. Patch test available upon request. This agreement shall be governed by the laws of the State of Ohio. By paying the down payment you are agreeing to all that is set forth in this legally binding contract. IN WITNESS WHEREOF, the parties hereto have executed this Agreement as of the day and year first above written.
