By filling up this form, I hereby agree and consent that my personal data provided in this form and all other forms related to my request pertaining to loan and its related matters may be collected, used, processed and disclosed by POLWEL for the purposes of processing my loan and its related matters, in accordance with the Personal Data Protection Act 2012 and all subsidiary legislation related thereto.

In respect of disclosure, I understand that POLWEL may disclose the personal data contained herein to third parties (which may be in or outside of Singapore) where necessary for the above-stated purposes.
