Vince Lombardi Cancer Foundation Grant Reporting Form

The Vince Lombardi Cancer Foundation is pleased to support the important work you are doing to help prevent cancer, provide the best care to those fighting it and find a cure. To better understand the impact of our funded programs, grant recipients are requested to submit a final grant report.

By submitting grant reporting information on your respective grant award, the Foundation can assess the quality and impact of funded programs, help improve our own grantmaking, and promote your innovative program. Information included in the report may be shared via email, social media, or with media outlets; if any data is confidential, please note that in the descriptions provided.

Grant reports are required to be eligible to apply for future Vince Lombardi Cancer Foundation funding. More information on the 2026 application will be available in early spring.
