ACKNOWLEDGEMENTOF RISK & REGISTRATION POLICY
I freely and voluntarily assume the rise and hazards in the nature of the program and activity and I understand and acknowledge that I as a participant may suffer personal and potentially serious injury due to unforeseeable or fortuitous event. Potential hazards may include but are not limited to the following: allergies, minor scrapes and bruises associated with creating art. Please be aware of potential slippery sections in the Mainstreet complex on the way from visitors parking or public road to my studio. These areas are usually taken care off. Please know that Liba Labik is not responsible for any potential injuries. I acknowledge that is my responsibility to advise Liba Labik of any potential health concerns that may affect my participation in the stated program. Students are responsible for cleaning their working space after each lesson. There will be $ 20, - penalty charged for leaving un-cleaned working space. Any damage made in the studio and/or in the townhouse # 11 6347 Bowview Rd NW Calgary will be charged adequately to the damage. Parking is ONLY allowed on the Mainstreet visitors parking lot or on the public road around the Mainstreet complex. Students can use ONLY front entrance of the townhouse. Using entrance through the backyard is prohibited. It is strictly prohibited to share the address of my home studio with anyone. The address is intended only for you as my student. Classes missed will not be refunded or carried over. Make up classes may be arranged, depending on the space in the studio. Please be aware that due to limited space of the studio, it will difficult. One month notice minimum is requested from student wishing to cancel classes to give me enough time to find a new student to fill his/her spot. Students are fully responsible for their own art supplies and artwork. Damaged or stolen art supplies or artworks will not be refunded. Please know that due to the limited space in the studio, there is no storage to keep your art supplies in the studio after the class. There are only few options to keep some of your materials. Students are responsible for wearing adequate clothing to art class. Use of turpentine and other harmful materials inside of the studio is strictly prohibited. Based on my understanding and acknowledgment as described I agree to register for private in person art lessons at LIBA Art Studio.
