Become a Mundi Project Ambassador!

Join our team of music advocates who volunteer their time, skills, and expertise to help Mundi Project fulfill its mission of breaking down socieconomic and generational barriers by providing quality music experiences for all!

Our ambassadors are essential to the work of Mundi Project. Opportunities include individual performances in a group setting with other ambassadors at community outreach events as well as teaching mentorships with our Harmony Hub classes for low-income music students.

Volunteer opportunities are sent on a regular basis via the Point mobile app and ambassadors are encouraged to sign up whenever possible. Registering as an ambassador does not require you to attend every event, however, we request that you complete a minimum of 5 hours per academic year.

Each year, the top 5 ambassadors with the most volunteer hours will be awarded the Lieutenant Governor Volunteer Recognition Award.

The work that ambassadors do is always voluntary and never considered a form of employment.

Ambassador Identity/Contact Information

Please tell us the names, ages, and pronouns of each ambassador below. You can register one ambassador or multiple ambassadors. The demographical information (BIPOC, Race/Ethnicity, LGBTQ+, Language Preference) is used exclusively for statistical reporting to national and regional foundations that support Mundi Project as well as internal program evaluation. While the entry requires a response, you may respond N/A if you prefer not to report this information.

By signing and submitting this form, I (or if I am a parent, I agree on behalf of my child) to the following:

I am/they are volunteering my/their time, without promise or expectation of compensation in any form, to Mundi Project upon my/their own free will, without duress, coercion or promise of benefit.

I am volunteering my/their time to Mundi Project because doing so is meaningful to me/them personally and allows me/them to provide charitable services and public benefit.

I/they understand that I am/they are not an employee and volunteering my/their time will not make me/them an employee of Mundi Project. In volunteering, I am/they are not being asked to volunteer full-time or displace a regularly employed employee.

General Liability Waiver               

As parent and/or legal guardian, I remain legally responsible for any personal actions taken by the below named minor (“Ambassador”). I agree on behalf of myself, my child named herein, or our heirs, successors, and assigns, to hold harmless and defend Mundi Project and its officers, directors, employees, chaperones and agents, and any organizational partners, their employees and agents, chaperones, or representatives associated with the event, from any claim arising from or in connection with my child attending the event or in connection with any illness or injury (including death) or cost of medical treatment in connection therewith, and I agree to compensate Mundi Project, its officers, directors and agents, and any organizational partner, their employees and agents and chaperones, or representative associated with the event for reasonable attorney’s fees and expenses which may incur in any action brought against them as a result of such injury or damage, unless such claim arises from the negligence of Mundi Project or their organizational partners.                                       

 

COVID-19 Waiver 

If your child(ren)/dependents or immediate family members exhibit any symptoms of COVID-19 (including cold, flu, fever, cough, or allergy symptoms) please refrain from allowing them to attend the event(s). You, as the parent/guardian of your child/dependent are freely choosing to allow them to attend the event(s), knowing the health risks present with the current environment and will hold harmless Mundi Project, its officers, directors, employees and agents, and any organizational partners, its employees and agents, chaperones, or representatives associated with the event, from any COVID-19 related claim arising from or in connection with your child/dependent attending the event.   

               

General Medical Matters and Emergency Medical Treatment 

I hereby warrant that to the best of my knowledge, my child is in good health and I assume all responsibility for the health of my child. In the event of an emergency, I hereby give permission for Mundi Project staff to call 911 and have my child transported via ambulance to a hospital for emergency medical or surgical treatment. I wish to be advised prior to any further treatment by the hospital or doctor.

Media Release 

I give permission to Mundi Project to publish photos and videos obtained of programming participants, including Zoom recordings or other screenshots, on its website, social media, and any other publications or platforms. I understand that the participant’s name and age may appear with the picture.

I release and discharge Mundi Project and its officers, employees, agents, and volunteers from and against any and all damages, complaints, costs, and fees arising from said use of such photo(s) and video(s). I agree to refrain from instituting, pressing, or in any way aiding any claim, demand, action or cause of action for damages, costs, compensation, or fees against the same in connection with such use.

Agreeing to the Media Release Form does not imply that the photos or video will be published; it simply indicates that Mundi Project has permission to do so in the event they are needed.
