In order to help me streamline the booking process, please fill out any relevant information I may need for your appointment below. If there is a question you do not understand or are not sure how to answer, give a best guess and let me know at the end how I can help! Once this has been filled out you will be automatically put onto a wait list. From there, you will be contacted VIA EMAIL to start the booking process. Once you place a deposit down on an appointment, that deposit is nonrefundable as a courtesy to my time. If an in-person consultation is permitted based on the information I have received, I will assist you in scheduling one. My in person consultation fee is $50, and this is to ensure that my time is respected and you receive all the information needed regarding your tattoo. This process guarantees that we have a solid plan moving forward for your piece. Not all inquiries may need an in-person consultation, but the option will be available to you no matter what. Please be advised that I will try to answer to all request forms within 72 hours; However, I am one person, and there may be times that it takes me a bit longer to contact you. Your request is important to me, and I appreciate your patience!
