I understand that if I am asking for RDK9 to pull a dog to foster that is not yet out of the shelter, that I will be responsible for said dog if they agree to pull & I will not abandon said dog once pulled. I understand if I do, then I will be liable for all costs associated with the dog & a $500 fine.*

I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any Regional Deployed K9 Inc animal fostered by me.

I certify that the above information is correct, and I understand that the information will be verified.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless RDK9 and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog.

I accept full responsibility for the dog(s) actions at all times, and release RDK9 from any liabilities or damages that may be incurred because of fostering such dog(s).

I agree to have RDK9 complete reference call checks and conduct a home visit inspection to be able to approve my foster application.

I agree that if I'm unable to foster the dog(s) anymore that I will return the dog(s) to RDK9 and give RDK9 a 3 week period to try and find another suitable foster for the dog(s). I also agree to share the cost of transport if requested by RDK9 for the return of the fostered animal if I cannot keep it. I also agree to pay for 3 weeks of boarding cost if I cannot hold the foster for the 3 week period.

I agree that all animals in foster will be taken care of properly and be given the adequate daily care required ie (but not limited to) food, water, exercise, vetting, monthly preventatives, proper housing ie not on a chain etc.

I agree to not rehome any canine under foster without the consent of RDK9 as well as any adoption fees collected will go directly to RDK9.

I agree that no canine in foster will be abandoned at a shelter for any reason.

I agree to send pictures, videos, and updates twice a week of dogs in foster so that RDK9 may adequately post the canines in need of homes and update potential adopters.

I agree to direct all possible adopters directly to RDK9 who will handle the adoption process in its entirety.

I agree that I will make myself available to meet potential adopters as needed within a 48 hour notice given by RDK9.

I agree that I will not euthanize any animal in foster without the consent of RDK9.

I agree that if I block RDK9 on social media or phone I will be assumed to have adopted foster animals and will be responsible for all their costs along with reimbursing RDK9 for money spent until that point.

I understand that should I wish to adopt an animal I am fostering I will contact RDK9 immediately as to inform them of the decision so that they do not continue to attempt to place the animal and I understand I will be responsible for all of the animal's costs as well as a reduced adoption fee.

I agree that I understand that the fostered animal may have come from a high kill shelter and there are no guarantees given by RDK9 regarding breed, temperament, health, etc.

I agree to seek immediate medical attention for the fostered animal should they need it and will submit any invoices given by the veterinary hospital if I wish to seek reimbursement of any kind. I agree that I will not aallow any treatment that I will seek reimbursement for prior to being given permission by RDK9. RDK9 will do their best to reimburse medical expenses within a timely manner providing an invoice is submitted with a detailed list of expenses incurred and the contact information for the veterinary hospital and that the veterinary hospital calls RDK9 at their main number before any elected procedures ie blood work, spay, neuter, X-Ray, surgeries, etc are performed and that RDK9 agrees to any treatments being performed with said vet. I understand if I do not received permission and agreement that costs will be reimbursed by RDK9 then I will be responsible for any medical treatment I allow.

I understand I will be liable for any medical bills that ensue due to improper containment or other situations that could have been prevented.

I understand that I am responsible for the daily costs of the fostered animal such as food, water, crate, toys, collars, leashes etc.

I agree to keep the microchip registered to RDK9 as the primary contact at all times.

I understand that if I do not abide by this contract that I will lose my right to foster in the future, will have the animal confiscated, and could potentially face legal action taken against me.
