      This form is not to be submitted after 9.00am on Tuesday for a weekend match or within three clear working days for a midweek match.

      When you submit this form you will receive a confirmation email and the request will be sent to the opposition club contact.

      If the oppostion club accepts the amendment request you will receive an email and the request will be sent to ESFA for final processing.

      If the opposition club declines the amendment request you will receive an email and no action will be taken by ESFA.

      • Fine of up to $200; and
      • Pay all appropriate referee and assistant referee fees; and
      • Pay all ground hire costs.
