Lip BlushIs a semi-permanent tattoo that can enhance the color and shape of your lips giving the impression of more fullness. This technique is done by depositing color ink into your lips.

Lip Neutralization This is the process of lightening dark lip tissue. This technique can take from 1 to 3 sessions. Those who have excessive lip hyperpigmentation are not a good candidate due to the risk of ending up with even darker lips.

This form is designed to give you the information you need to make an informed choice of whether or not to undergo a permanent cosmetic application.

Although permanent cosmetic tattooing is effective in most cases, no guarantee can be made that a specific client will benefit from the procedure.

This is the process of implanting pigment into the dermal layer of skin and is a form of tattooing used for the purpose of permanent cosmetic makeup and skin imperfection camouflage.

All instruments used are new, sealed, sterilized and disposable. Extreme sanitary measures are used.

Topical anesthetics are used to numb the area of skin to be performed on. Dermacaine, Tetracaine & Benzocaine in a cream form and Lidocaine in gel form are typically used.

Generally, the results are excellent. However, a perfect result is not often a realistic expectation after the first application. I will be doing a perfecting application after healing has been completed within 30-45 days.

The treated area(s) initially may become swollen and/or slightly red. Bruising may occur with eyeliner and lip procedures. Healing of all procedures will take from 15-30 days. Color often changes/improves after 2 weeks.

You will likely experience exfoliating of the skin, redness, itching and possible scabbing. Do not pick or remove this epithelial crust. Doing so will result in loss of pigment.

You will use an aftercare product that I provide.

The pigment color initially will be more vibrant compared to the end result. This process is called oxidation. Usually within 5-7 days the color will fade 20-60%, soften and look more natural. Although the pigment is permanent, the color will fade somewhat over time and will likely need to be retouched.

Some possible risks, hazards or complications are:
* Pain: There is little to slight pain involved in the procedure once the topicals have been applied and have taken effect. Lip procedures can involve more discomfort.
* Infection: Infection is very unusual. The areas treated must be kept clean and you must wash
your hands after handling anything, including pets. Do not touch the area with your bare fingers for the first week following the procedure. This may cause contamination. Use only a Q-tip or gauze pad to cleanse the area. See “After Care” sheet for instructions.
* Uneven Pigmentation: This may result from poor healing, infection, improper care of treated areas, bleeding, etc. Your follow up appointment will determine the reason and resolution. A retouch will be needed or assessed at your follow up appointment.
* Poor Contour/Asymmetry: Every effort will be made to apply the pigment in a fashion that is similar to your usual makeup application contours or the agreed upon shape. Our perfecting retouch appointment will give the finishing balance.
* Excessive Swelling or Bruising: Some individuals may have more than usual swelling or bruising. This will resolve spontaneously within a day or so. Ice pack applications will help. Some clients have little to no swelling or bruising at all.
* Eye Exposure: There is a minuscule risk of eye injury when an eyeliner procedure is performed. To avoid a corneal abrasion, Celluvisc, a thick eye drop, is used to protect the eye prior to the
procedure to avoid the risk of any topical or pigment damaging or irritating the eye. Refresh Plus Eye drops are used to cleanse and flush the eye after the procedure is complete.
* Anesthesia: Dermacaine topical cream and Lidocaine with Epinephrine gel and/or cream are used to numb the skin to be treated. If you are allergic to any of these, please inform before we start the procedure.

* Allergic reaction: A minute possibility of an allergic reaction may occur; therefore you may
take a 5-7 day patch test to determine this. It will This form is designed to give you the information you need to make an informed choice of whether or not to undergo a permanent cosmetic application.

* MRI: Because pigments used in permanent cosmetic procedures contain inert oxides, a low-level magnet may be required if you need to be scanned by a MRI machine. You must inform the MRI technician of any tattoos or permanent cosmetics.
* Fever Blisters: If you are prone to cold sores or fever blisters, herpes simplex2, there is a High Probability that you WILL get them during Lip procedures. It is advised that you call your doctor for a prescription of anti-viral medication to help prevent this from occurring. L-Lysine DOES NOT PREVENT COLD
SORES. It is your responsibility to get the prescription.
* Donating Blood: You may not be able to donate blood for up to one year after having a tattoo procedure. Check with your local Blood Bank or Health Dept. The alternative to this procedure is to use topical cosmetics and not undergo the Permanent Cosmetic procedure.

I am over the age of 18, am not under the influence of drug or alcohol and desire to receive the indicated permanent cosmetic procedure. The general nature of cosmetic tattooing as well as the specific procedure to be performed has been explained to me.

I have been informed of the nature, risks, and possible complications and consequences of permanent skin pigmentation. I understand that permanent skin pigmentation procedures carry with it known and unknown complications and consequences associated with this type of cosmetic procedure, including but not limited to infection, allergic reaction, scarring, inconsistent color, spreading, fanning, or fading of pigment. I have read information above and I comply. I understand that the procedure may require topical anesthesia mentioned above of possible complications and I will advice of any allergy reaction prior to the procedure. I give my consent to have the application of the mention topical anesthesia.

I am aware that my lips/eyebrows/eyeliner may appear darker by up to 50% during the first week following treatment. I understand and accept that each treatment is a process requiring multiple applications of pigments to achieve desirable results, and the 100% success cannot be guarantee as every skin type is different. I understand that is why I need to return for a touch up 4-8 weeks later and/or more touch up.I’m aware and understand that in color correction and cover up cases the pigments can go away because of the previous work. The Artist explained that not all correction or cover up are workable and there’s always risk of loosing the implemented pigment. I voluntarily accept the risk and continue the treatment. If the pigment goes away the Artist can refuse to do the touch up(s) or can recommend future services.

I understand and agree that is my responsibility to confirm when I’m happy with the pre-drawn shape and colour choice prior to the treatment. It is my responsibility to advise if I’m not happy before starting the treatment. I request the permanent skin pigmentation procedure(s) and accept the permanence of the procedure as well as the possible complications and consequences of the procedures.

I understand and agree that after my initial appointment of Permanent Makeup, I will be coming back in a 6-8 period time for a touch up and this will have an additional cost, touch up(s) can take up to 3 sessions to get the desire results (ALL TOUCH UPS ARE PAYABLES, NO EXCEPTIONS).I understand and agree that If after my initial appointment I am not happy with the results once my eyebrows/Lips/eyeliner are healed I will address the Artist with respect, to see what will be the next step. I will not make any comment in social media or do bad reviews in the different platforms because I understand the work is not 100% guaranteed and I’m doing it freely and voluntarily.

I understand that having any skin treatment, laser hair removal, plastic surgery, or other skin-altering procedure(s) may result in adverse changes to my permanent cosmetic. I acknowledge some of these potential adverse changes may not be corrected.

I have received previous and post-procedure (s) instructions and I will strictly; adhere to such instructions. I understand that my failure to do so may jeopardize; my chances for a successful procedure; if I am on any medications for; depression or any other mood-altering prescription. In case of infection I will use Antibiotics ointment and consult with my doctor.

I have read the above information and understand that following pre care recommendation is mandatory if I desire lipliner or full lip color procedures. I will advise my technician if I have ever had cold sores, I will consult with and strictly follow my doctor instructions before contemplating any permanent cosmetic procedure(s) around my lips.

I understand that if anything changes and something happens after the procedure, I will negotiate with the technician to resolve the problem. I will not file the complaint nor sue the technician.

I understand that each result is individual and it can vary from person to person. I will not use social or any other media to express my dissatisfaction about the results and I will not do any comment that it could impact the artist career or the artist business. I understand that if I do that it may have legal implications. Any concern that I might have after the procedure will be discussed directly with the artist.

I understand that the taking of before and after photographs of the set procedures are a condition of such procedures. I certify that I have read above paragraph, and I have had explained to my understanding this consent and procedures permit I accept full responsibility for the decision to have this photos and videos taken.

I allow The Brows of Frida to take or capture my photos and videos.

I understand that all photos and videos that will be taken in this activity are copyrighted by The Brows of Frida.

I understand that I will/ will not receive any monetary compensation.

I authorize The Brows of Frida to distribute and reproduce the materials for the following purposes: Portfolio showcase, advertising, marketing, branding, educational, digital promotions, internet videos, online courses, media, other commercial or non-commercial purposes

I grant The Brows of Frida to use my photos and videos on Youtube, Vimeo, Facebook, Twitter, Instagram, and other social media platform.

I release The Brows of Frida from all liability and obligations from any claim for injury, illnesses, claims, or demands.

I do not permit The Brows of Frida to use these materials that can harm my reputation or others.

I the undersigned, hereby agreed that we have read this agreement and bounded by it.

If I don’t want to be in social media I will address it with the artist.

By completing and signing this form I agree that I have read the consent form and the Photo and Video Release form. I acknowledge that the fees, once charged are non-refundable. I understand that a $50.00 fee will be charge for late cancelation or no-show.
