This questionnaire is designed to help the For the Love of Dog Rescue team and yourself determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. We reserve the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!

All dogs/puppies adopted from FTLOD Rescue come with age appropriate distemper/rabies vaccines as well as microchipping & spay/neuter surgery pre-paid if not alreay completed at the time of adoption.* FTLOD Rescue offers lifetime support to adopting families and encourages all adopters to contact the rescue with any questions regarding their new family member.

*FTLOD rescue does not condone premature spay and neuter surgeries and encourages all adopters to wait until their new family member is 6 MONTHS OF AGE OR OLDER before altering.
