In order for us to provide the best service possible and make the best selections for your monthly subscription box, we need to get to know you a little better!

  I hereby authorize Haute Boutique, LLC in Round Rock, Texas to charge my credit or debit card in the amount of $49.95 per month on the 20th of every month (or thereafter) for the Haute Box monthly subscription service or $69.95 per month on the 20th of every month (or thereafter) for the Hive Membership. Should my draft be declined or come back for insufficient funds, I understand that my subscription will be cancelled, all unused HIVE Box rewards will be voided and I will be charged a $25 early cancellation fee. 

  I further verify and attest that the card information provided is true and under my own personal identity. I am not unlawfully using or compromising information that does not belong to me. 

  I understand that if I am signing up before the 20th of the month, that I will be charged on the 20th for the next months box. 

• All HIVE/Haute Box subscribers knowingly authorize GALA4, LLC in Round Rock, Texas to run their card on file for the monthly subscription fee of $49.95 (Haute Box only) or $69.95 (Hive Membership).
• All customers must complete the enrollment form and credit card authorization.
• If at any time your draft is declined or comes back for insufficient funds you will be given 48 hours to complete your payment before your subscription is canceled and all remaining HIVE Box rewards are voided.
• If you cancel without a 45 day written notice all Haute Box rewards are voided and you will be charged a $25 early cancellation fee.
• Cancellation can be made at any time with a written 45-day notice. In fairness to all subscribers, there will be no exception to this policy.
• HIVE/Haute box payment will draft on the 20th of every month and your box will ship no later than the 28th of the following month.
• If you live within a 20 mile radius of 503 E. Main St, Round Rock, Tx, your box will automatically be delivered to the address on file. If you ever need to update this address or change to local pickup please email info@hautellc.com.
• HIVE/Haute boxes that are shipped will be shipped with Priority Mail 1-3 day delivery. We are not responsible for delays caused by the United States Postal Service.
• Customers acknowledge that any allergies to metals, foods, etc that could be included in the monthly HIVE/Haute Box are noted on their customer questionnaire. Haute Boutique will not be liable for any allergies or medical conditions that are not disclosed.
• If/when "gifting" a HIVE/Haute Box to another person, we must have a customer questionnaire completed and signed on their behalf. If you are signing for that person you will, by default, assume responsibility for their draft, terms, and conditions.
