Requirements
New Students:
- Fill out the admission form for new students.
- Wait for your exam and interview schedule.
- Pass the following requirements once you finish the enrollment process. Put them in a brown envelope and write your full name and grade level.
  - Form 138 (Report Card)
  - SF10 (Permanent Record)
  - Good Moral Character Certicate
  - Photocopy of PSA Birth Certificate
  - Photocopy of Baptismal Certificate
  - Photocopy of Parent's Marriage Certificate (If applicable)
  - 2 Pcs. 2 X 2 I. D. Picture
Old Students
- Form 138 (Report Card) - You may forward this at the end of the school year.

- Office is open from 9:00 AM to 4:00 PM (Mondays to Fridays) and 9:00 AM - 12:NN (Saturdays)
- Health and Safety protocols are strictly implemented.
- Bring your own ballpen.
