      • Pass the following requirements once you finish the enrollment process. Put them in a brown envelope and write your full name and grade level.
        • Form 138 (Report Card)
        • SF10 (Permanent Record)
        • Good Moral Character Certicate
        • Photocopy of PSA Birth Certificate
        • Photocopy of Baptismal Certificate
        • Photocopy of Parent's Marriage Certificate (If applicable)
        • 2 Pcs. 2 X 2 I. D. Picture
