Liability Waiver
CEL Studios, LLC DBA Bravo Dance Centre reserves the right to drop my student trom any and all classes tor
any reason whatsoever, including but not limited to:
1. Disruptive behavior
2. Excessive tardiness or absence. or
3. Non-Payment of accounts
I understand and agree that my student will be participating in a physical activity at Bravo, and as such I realize there is an element of physical risk involved.
I understand and agree that in the event of an accident or illness involving my student, reasonable attempts will be made to notify me immediately. However
in the event that I cannot be reached I herebv desisnate: Name phone. an emergency
contact.
I hereby authorize CEL Studios. LLC DBA Bravo Dance Centre' to take any and all steps it deems necessary to make available and provide medical attention and care, including physicians, hospital, or any other services. Furthermore. CEL Studios. LLC shall have full authoritv and discretion to make medical and healthcare
decisions for my student. I further agree that the cost of the medical expenses shall be borne exclusively by me. I understand and agree that any limitation in ability to participate due to a medical condition MUST be noted on a separate page and hand-delivered is a warranty by me that no medical conditions exist.
Additionally, please note that the following rules will be strictly enforced:
1. Any student that drops a class after October 15th will still be responsible for FULL payment of any and all costumes.
2. Any student who is not wearing proper dance wear will not be allowed to participate in class. Please make sure your student wears appropriate attire, no matter their age. This has been a tremendous problem in the past out of high school students
3. Attendance at dress rehearsals) is required to participate in recital
4. Students should be brought to the studio NO MORE THAT 10 MINUTES before their class and picked up promptly after their class ends. This is very important because the staff gets to the studio early to begin the business day, work on choreography, and other non-teaching job duties and cannot be responsible for watching your student it they are dropped off anv earlier. Also. the staff will often arrive 30 minutes early and then might need to leave to run an errand. We cannot be responsible, and we appreciate you abiding by this policy.
5. Please understand unpaid accounts have a direct effect on your student. A student will not be allowed to dance it their account is past due.
