    $190 per One-Week Session, except for Rocking Rainforest, which is $155 for the week.

                          • By registering your child, you are agreeing to the following:

                            • The information I have provided on this form is correct as far as I know

                            • If you wish to cancel your camp registration, you must notify Arts on the Horizon via email at least 4 weeks prior to the start of the registered session in order to receive a tuition refund in the form of an account credit to be used toward future Arts on the Horizon education classes or camps. If you cancel your registration less than 4 weeks prior to the start of the camp, you will receive 50% of your registration in the form of a credit. All credits are valid for one year after the date of issue.

                            • Low Enrollment/Cancellation Policy: A minimum enrollment is required to run each program session. Sessions with low enrollment prior to the start date may be subject to cancellation. Any registrations that are cancelled due to low enrollment will be refunded in full.

                            • Although every effort is made to provide a safe environment, I recognize there is always a risk of accident or injury. I agree to be responsible for any medical expense incurred from illness or injury as a result of my child's participation in Arts on the Horizon’s classes or camps. I understand that children are expected to be covered by their own, or their parent’s or guardian’s, insurance. Accordingly, I hereby release Arts on the Horizon from any and all liability, claims, or damages arising out of my child’s participation at Arts in the Horizon’s classes or camps.

                            • Additionally, I authorize Arts on the Horizon to administer necessary treatment for my child. I hereby further release Arts on the Horizon from any and all liability, claims, or damages that may result from such administration of treatment.
