You will need to fill out one registration per child. At the end of the regsitration form you will be taken to our payment portal. Once you have completed the regsitration and payment you can start a new registration form for the next child.

REGISTRATION PAYMENT
By submitting the form, I, as the parent/guardian of the registrant, I hereby give consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may given under whatever conditions are necessary to preserve the life, limb, or well-being of registrant. By submitting the form, I, the parent/guardian of the registrant, a minor, agree that I and the registrant will abide by the rules of PAC GLOBAL FUTBOL CLUB, its affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with soccer and in consideration for PAC GLOBAL FUTBOL CLUB, accepting the registrant for its soccer programs and activities (the "Programs"). I hereby release, discharge and/or otherwise indemnify PAC GLOBAL FUTBOL CLUB, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of fields and facilities utilized for the Programs, against any claimby or on behalf of the registrant as a result of the registrant's participation in the Programs and/or being transported to or from the same, which transportation I hereby authorize.

PAYMENT INFORMATION

PAC FC - Academy Monthly Fee is $150.00(tax) due on the 10th of each month. The registration below includes your first monthly fee, club fees, and 3 practice shirts. Payment(s) will be charged on the credit card provided by photo after you submit this form. If you need to change credit card info, please call main office or email. If you need to cancel your monthly fees, please give a 30-day notice in writing to pacglobalacademy@gmail.com to prevent future charges. Failure to submit a 30 day written notice will result in your next scheduled monthly fee to be charged. If your payment should decline or not process on the 10th, please note that a $10.00 late will be applied to your total. If you fall behind on your payments then the following month on the 10th both the pass due fee + late fee and the current month’s fee will be due. No refunds are available on any amounts. COVID-19 Liability Release Wavier
The world Health Organization has declared the novel Coronavirus (COVID-19) a worldwide pandemic. Due to its capcity to transmit from person-to-person through respiratory droplets, the government has set recommendations, guidlines, and some prohibitions which PAC GLOBAL ACADEMY and PAC GLOBAL FUTBOL CLUB adheres to comply.
In consideration of my participation in the forgoing, the undersigned acknowledge and agree to the following:*

I am aware of the existence of the risk on my physical appearance to the center and my participation to the activity of the soccer at PAC GLOBAL ACADEMY that may cause injury or illness such as, but not limited to influenza, MSA, or COVID-19 that may lead to paralysis or death.I have not experienced symptoms that of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease within the last 14 days.I have not, nor any member of my household, visit any area within the United States that was reported to be highly affected by COVID-19, in the last 30 days.I have not been, nor any member(s) of my household, diagnosed to be infected of COVID-19 virus within the last 30 days.
Following the pronouncements above I hereby declare the following:*

I am fully and personally responsible for my own safety and actions while and during may participation and I recognize that I may be in any case be at risk of contracting COVID-19.With full knowledge of the risks involved, I hereby release, waive, discharge PAC GLOBAL ACADEMY, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss. damage, injury, or death, that may be sustained by me related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to COVID-19.I agree to indemnify, defend, and hold harmless PAC GLOBAL ACADEMY from and against any and all costs, expenses, damages, lawsuits, and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any of the released party due to injury, loss, or death from or related to COVID-19.
By siging below I acknowledge that I have read the foregoing Liability Release Waiver and understand its contents; that I am at least (18) years old or the parent/guardian of my child and fully competent to give my consent; that I have been sufficiently informed of the risk involved and give my voluntary consent in siging it as my own free act and deed; that I give my voluntary consent in sgining this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation. This waiver will remain effective until laws and mandates relevant to COVID-19 are lifted.

In consideration of being permitted to participate in any in this indoor/outdoor facility, PAC GLOBAL ACADEMY CENTER, I, for myself for personal representatives, assigns, heirs, and next to kin:*

Acknowledge, agree and represent that I understand the nature of Soccer activities and that I am or my child(s) are qualified, in good health, and in proper physical condition to participate in such activity. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the activity.I fully understand that (a) soccer activities involve risk and dangers of SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH ("RISK"); (b) these risk and dangers may be caused by my or their own actions or inaction, the actions and inactions of others participating in the activity, the condition in which the activity takes place, or the NEGLIGENCE OF THE "RELEASEES" NAMED BELOW: (c) there may be OTHER RISK SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME SUCH RISK AND ALL RESPONSIBILITY FOR LOSSES ,COST, AND DAMAGES. I incur as results of my participation or that of a minor in activity. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE the PAC GLOBAL ACADEMY & PAC GLOBAL FUTBOL CLUB their respective administrators, directors, agents, officers, members, volunteers and employees, other participants, parents, any sponsors, advertisers, and if applicable, owner and lessors of premises on which the activity takes place, (each considered one of the "RELEASEES'" herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHILE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESUCE OPERATIONS AND I FURTHER AGREE that if, despite this RELEASE AND WAVIER OF LIABILITY, ASSUMPTIONS RISK, AND INDEMNITY AGREEMENT, I or anyone on my behalf, makes a claim against any of the RELEASERS. I WILL INDEMNITY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASERS from any litigations expenses, attorney fees, loss, liability, damage, or cost, which may incur as the results of such claim.
I read this agreement, fully understand its terms, understand that I have given up substantial rights by signing it and have signed it freely and without inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agrre if any portion of this agrement is held to be invaild the balance, notwithstanding, shall continue in full force and effect.

I hereby grant PAC GLOBAL ACADEMY AND PAC GLOBAL FUTBOL CLUB the permission to use photographs/media for any legal use, including but not limited to: print publications, online publications, presentations, copyright purposes, illustration, advertising, websites and social media content.*

PAC GLOBAL ACADEMY FIELD RULES:

1. Parking is located in LOT B - LOT D, inclduing player drop off.

2. No parents or siblings on the fields at any time before, during or after practice.

3. ALL trash must be thrown away using the trash cans are provided at all gate entrances.

4. NO scooters, bicycles, skates or skateboards.

5. NO gum, smoking, alcohol or drugs.

6. NO food of any kind on the fields.

Participants and spectators assume all risk of injury, damage or loss sustained on premises.

By sigining you agree to all field rules of PAC GLOBAL ACADEMY and agree to all assumed risk of injury, damage or loss sustained on premises.
PAC GLOBAL ACADEMY CONSENT OF LIABILITY:

1. I am not aware of any injury, illness or other health issues that would resrtict or limit my ability to play all levels of competitive soccer.

2. I agree to assume all the risk and expenses due to an injury that may occur as a result of my childs involvement in competitive soccer practices, games, tournaments and/or to and from said activities at PAC GLOBAL ACADEMY.

3. I agree to hold PAC GLOBAL ACADEMY or anyone on its behalf either as a Coach, a coaching assistant or staff member harmless in the event of an injury while participating under the supervision of the above.

4. I agree to be held responsible in the event that my child/I may cause damage to the complex facilities.
