By signing this document, I acknowledge and agree to the following:

Assumption of Risk:

I understand that participating in physical exercise, weightlifting, and using gym equipment involves risks of injury, including serious injury or death. I voluntarily choose to participate and accept full responsibility for any injury or harm that may occur.

Release of Liability:

I release and discharge Omni Fitness, its owners, coaches, staff, and representatives from all liability, claims, demands, or causes of action for any injury, damage, or loss that I may suffer as a result of my participation, including from negligence, accidents, equipment failure, or other walk-in gym users.

Gym Rules:

I agree to follow all gym rules and respect all equipment and other gym users.
I understand that damaging equipment through misuse or deliberate action may result in charges to repair/replace and a ban from returning.
I agree to notify staff immediately if any equipment is broken or unsafe.
Failure to report broken equipment may result in termination of access.

Medical Clearance:
I confirm I am physically able to participate and have no medical conditions that would prevent safe involvement in gym activities. I understand it is my responsibility to seek medical clearance if needed.

No Supervision:
I understand that during my casual visit, there may be no coach supervision and I am responsible for my own safety and equipment use.

Photography/Video Consent
I consent to the use of any photos or videos taken during my visit for promotional purposes.

GYM ACCESS RULES & CONDITIONS (CASUAL & MEMBERS)
- By signing this form, I acknowledge and agree to the following conditions of using Omni Fitness:
  
  I will only access the facility between the hours of 5:00 AM and 11:00 PM.
  I understand that the last person to leave is responsible for turning off all lights, fans, and music. A closing checklist will be posted on the door for reference.
  I will always put away my weights and equipment after use.
  I understand that it is strictly against the rules to bring any non-members into the facility. Doing so may result in membership cancellation and fines.
  I agree to only park in the designated Omni Fitness parking area or on the street. No parking in neighboring business lots.
  I understand there are nearby businesses, and I will keep music at a reasonable level to avoid disruption.
  I understand that roller door access is restricted to staff hours only.
