This is to ensure that the dog's presence in your residence is compliant with your landlord's regulations or lease terms. 
    Applicants Considering an Emotional Support Animal (ESA) or Service Animal: If you're considering adopting a dog for emotional support or as a service animal and you're currently residing in a rented property, please note that we require applicants to possess a letter from a licensed medical provider explicitly stating the need for a support or service animal for your well-being. During the application process, you'll be asked to provide proof of this letter. Kindly ensure this document is available when applying for adoption.

Therefore, please refrain from submitting an adoption application with Lexington Pit Crew if the intent is to try to talk someone into adopting, if the decision hinges solely on meeting the dog in person, or if it's meant as a surprise for someone else. Adopting a dog is a commitment that requires thoughtful consideration and involvement from all household members.

    I hereby certify that all information provided on this application is truthful and accurate. I understand that any misrepresentations may lead to disapproval of my application and/or the immediate removal of the adopted dog from my possession. Additionally, I certify that I have never been convicted or charged with any form of animal abuse, neglect or animal hoarding.

    I consent to allowing a representative of Lexington Pit Crew, Inc to contact my veterinarian's office as a reference. I also agree to a mandatory home visit conducted by a representative of Lexington Pit Crew, Inc prior to adoption of a dog. I understand that this is process is crucial in ensuring the placement on Lexington Pit Crew, Inc's dogs in safe and suitable environments. I acknowledge that it is required and a non-negotiable step in Lexington Pit Crew, Inc's adoption process, which is aimed at the well-being and safety of their rescue dogs.

    I acknowledge that upon approval of my adoption application, I agree to the payment of a non-refundable adoption fee and agree that I must willingly sign a trial contract prior to taking possession of the dog I was approved for. Additionally, I acknowledge that I must willingly sign an adoption contract at the time of adoption. 

    I acknowledge that prior to adoption, all dogs from Lexington Pit Crew, Inc are spayed/neutered, undergo heartworm testing, fecal testing, and are current on vaccinations.
